They were divided into a survival group (113 cases) and a death group (23 cases) according to whether the patients died during treatment. The general clinical data and laboratory test indexes of the two groups were compared to analyze the risk factors affecting the death of pelvic fracture patients. Receiver operator characteristic (ROC) curves were plotted, and the area under the curve was calculated.</p><p><strong>Results: </strong>The two groups were significantly different in aspects of age, systolic blood pressure, rapid emergency medicine score, injury severity score (ISS), sequential organ failure assessment (SOFA) score, prothrombin time, activated partial thromboplastin time, and percentages of patients with bleeding volume > 2,000 mL, shock index > 2, and minimum oxygenation index < 200 (P<0.05). Logistic regression analysis suggested that shock index > 2, ISS, SOFA score, and bleeding volume > 2,000 mL were independent risk factors for patient death. ROC analysis showed that the area under the curve for shock index > 2, ISS, SOFA score, and bleeding volume > 2,000 mL was 0.656, 0.732, 0.907, and 0.798, respectively.</p><p><strong>Conclusions: </strong>Severe shock, increased ISS and SOFA score, and bleeding are independent risk factors for death in patients with hemodynamically unstable pelvic fractures. 摘要

目的:分析骨盆骨折血流动力学不稳定患者治疗过程中发生死亡的相关危险因素。方法:本研究采用回顾性研究设计,选取2020年3月至2022年3月在华钢总医院住院的136例骨盆血流动力学不稳定骨折患者作为研究对象。根据患者在治疗过程中是否死亡分为生存组(113例)和死亡组(23例)。比较两组患者的一般临床资料和实验室检查指标,分析影响骨盆骨折患者死亡的危险因素。绘制受试者操作特征(Receiver operator characteristic, ROC)曲线,计算曲线下面积。结果:两组患者在年龄、收缩压、快速急诊医学评分、损伤严重程度评分(ISS)、顺序性器官功能衰竭评分(SOFA)、凝血酶原时间、活化部分凝血酶时间、出血量>0 000 mL、休克指数> 2、最小氧合指数< 200的患者比例(P 2)方面存在显著差异,ISS、SOFA评分、出血量> 2000 mL是患者死亡的独立危险因素。ROC分析显示,休克指数>2、ISS、SOFA评分、出血量> 2000 mL的曲线下面积分别为0.656、0.732、0.907、0.798。结论:严重休克、ISS和SOFA评分升高、出血是骨盆骨折血流动力学不稳定患者死亡的独立危险因素。在治疗具有这些因素的高危患者期间实施快速和有效的针对性管理是降低其死亡风险的关键。
