Gynecomastia develops when there is an increase in the ratio of estrogen to androgens. Underlying causes are plenty and include drugs, congenital and acquired disorders of androgen and estrogen production, various tumors, renal failure, cirrhosis of the liver, and thyrotoxicosis.
