Changes in mRNA detection rates were analyzed according to the time of blood sample collection, the surgical modality, and patient clinicopathological features.</p><p><strong>Results: </strong>mRNA expression rates before surgical resection did not differ between blood samples from the peripheral and inferior mesenteric veins. The detection rate for CEA and CK20 mRNA showed a tendency to increase after operative mobilization of the cancer-bearing bowel segment. Furthermore, the cumulative detection rates for CEA and CK20 mRNA increased significantly over the course of surgery (pre-mobilization vs. post-mobilization). The cumulative detection rate decreased significantly after surgical resection compared with the pre-operative rates. However, no significant difference was observed in the detection rates between different surgical modalities (laparoscopy vs. open surgery).</p><p><strong>Conclusion: </strong>The results of this study suggest that surgical manipulation has a negative influence on the dissemination of circulating tumor cells during operations on localized colorectal cancer. 摘要

目的:本研究的目的是评估局部结直肠癌循环肿瘤细胞分子标志物的检测与手术手法或手术方式的时间过程之间的关系。方法:选取2010年1月~ 6月42例乙状结肠、直肠癌患者的外周血及肠系膜下静脉标本。采集术前、术中(活动前、活动后)及术后标本。采用实时逆转录聚合酶链反应检测癌胚抗原(CEA) mRNA和细胞角蛋白-20 (CK20) mRNA。根据采血时间、手术方式及患者临床病理特征分析mRNA检出率的变化。结果:手术切除前肠系膜外周静脉和下肠系膜静脉血液样本的mRNA表达率无差异。CEA和CK20 mRNA的检出率在癌肠段手术动员后呈上升趋势。此外,CEA和CK20 mRNA的累积检出率在手术过程中显著增加(动员前与动员后)。术后累计检出率较术前明显下降。然而,不同手术方式(腹腔镜与开放手术)的检出率无显著差异。结论:本研究提示手术手法对局限性结直肠癌术中循环肿瘤细胞的播散有负面影响。然而,手术技术的类型不影响循环肿瘤细胞。

Influence of surgical manipulation and surgical modality on the molecular detection of circulating tumor cells from colorectal cancer.

Purpose: The aim of this study was to evaluate the relationship between the detection of circulating tumor cell molecular markers from localized colorectal cancer and the time-course of a surgical manipulation or surgical modality.
