A comparison of the presence or absence of neonatal complications based on the delivery type indicated a higher occurrence of RDS with cesarean deliveries (<i>P</i> = 0.025). The intubation and mechanical ventilation periods were not significantly longer in neonates delivered via cesarean section.</p><p><strong>Conclusions: </strong>Cesarean delivery is a risk factor for neonatal RDS in women with preterm premature rupture of membranes. 摘要

目的:本研究旨在阐明剖宫产对早产儿胎膜早破新生儿呼吸系统疾病的影响。方法:本回顾性研究纳入2009年1月至2014年12月妊娠23 ~ 33周分娩的早产胎膜早破妇女。比较剖宫产和顺产两种分娩方式的新生儿结局。主要终点为呼吸窘迫综合征(RDS)。新生儿插管和机械通气期是次要结局。倾向评分匹配用于比较剖宫产和阴道分娩病例的结果。结果:剖宫产101例,阴道分娩89例。基于分娩类型的新生儿并发症的存在或不存在的比较表明剖宫产的RDS发生率更高(P = 0.025)。剖宫产新生儿插管和机械通气时间无明显延长。结论:剖宫产是早产胎膜早破妇女新生儿RDS的危险因素。需要通过试验确定新生儿的长期预后,以进一步制定此类病例的最佳管理策略。
