Despite the exact prevalence of PAAs being unknown, it is thought to be present in about 1% of the elderly male population. Acute thrombus in a PAA can increase the risk of lower limb ischemia and amputation. We present a case documenting an incidental finding of an asymptomatic “floating” or pedunculated thrombus within a left PAA on duplex ultrasound.
