Yao, Wenqi Song","doi":"10.3760/CMA.J.CN101070-20190429-00362","DOIUrl":null,"url":null,"abstract":"Objective \nTo investigate the molecular characteristic and antimicrobial resistance of Staphyloco-ccus aureus (S.aureus) isolated from children with pneumonia, in order to provide the evidence for clinical diagnosis and treatment. \n \n \nMethod \ns The S.aureus strains isolated from children diagnosed as S.aureus pneumonia in the Beijing Children′s Hospital, Capital Medical University, between January 2016 and March 2017 were collected.Methicillin-resistant S.aureus(MRSA)and Methicillin-susceptible S.aureus(MSSA)were identified using the cefoxitin disc method and the detection of the mecA gene.S.aureus isolates were characterized by multilocus sequence typing (MLST) and Staphylococcal protein A (spa) typing, and MRSA strains were characterized by Staphylococcal chromosome cassette (SCCmec) typing.Besides, 21 superantigens (SAgs) genes, panton-valentine leucocidin (PVL) genes, adhesion genes fnbb and cna were detected by PCR; E-test was used to detect in vitro drug sensitivity of 14 antibiotics. \n \n \nResult \ns A total of 42 S.aureus were collected, and 21 isolates were MSSA, the same to MRSA, while MSSA strains had relatively dispersed typing, with ST25-t078 (14.2%) as the most common one.ST59-SCCmecⅣa-t437 (71.4%) were the most predominant clones of MRSA.Of the 42 isolates, 36 strains (85.7%) had at least 1 superantigen gene, and sek-seq (21.4%) was the main virulence genotype.The ratio of pvl in MRSA strains (52.3%)was significantly higher than that in MSSA strains (14.2%), while the detection rate of fnbB and cna in MRSA(9.5%, 9.5%) was significantly lower than that in MSSA(42.8%, 47.6%), and the differences were statistically significant (all P<0.05). 摘要

目的了解儿童肺炎分离金黄色葡萄球菌的分子特征及耐药性,为临床诊断和治疗提供依据。方法收集2016年1月至2017年3月首都医科大学附属北京儿童医院诊断为金黄色葡萄球菌肺炎的患儿分离的金黄色葡萄球菌菌株。采用头孢西丁片法对耐甲氧西林金黄色葡萄球菌(MRSA)和敏感金黄色葡萄球菌(MSSA)进行鉴定,并对mecA基因进行检测,对金黄色葡萄球菌进行多位点序列分型(MLST)和葡萄球菌蛋白A (spa)分型,对MRSA菌株进行葡萄球菌染色体盒(SCCmec)分型。PCR检测到21个超抗原(sag)基因、PVL基因、粘附基因fnbb和cna;采用E-test法检测14种抗生素的体外药敏。结果共采集到42株金黄色葡萄球菌,其中21株为MSSA,与MRSA相同,而MSSA菌株分型相对分散,以ST25-t078(14.2%)最为常见。ST59-SCCmecⅣa-t437(71.4%)是MRSA的最优势克隆。42株分离株中有36株(85.7%)具有至少1个超抗原基因,其中sek-seq为主要毒力型(21.4%)。MRSA株pvl检出率(52.3%)显著高于MSSA株(14.2%),MRSA株fnbB、cna检出率(9.5%、9.5%)显著低于MSSA株(42.8%、47.6%),差异均有统计学意义(P<0.05)。94%(38/42株)的金黄色葡萄球菌分离株具有多重耐药。结论金黄色葡萄球菌肺炎患儿MRSA检出率较高,其优势克隆型为ST59-SCCmecⅣa-t437。金黄色葡萄球菌超抗原基因的前价较高,耐多药率较高。MRSA菌株通常携带pvl基因,而MSSA菌株通常携带fnbB和cna基因。关键词:肺炎;孩子;金黄色葡萄球菌;分子特征;毒性;抗菌药物敏感性
