Methods: The present double-blind, randomized clinical trial was conducted on 70 children aged 1 - 6 years that were candidates for lower abdominal surgery. The patients were assigned to two groups, each consisting of 35 patients. Before induction of anesthesia, 0.02 mg/kg of atropine and 0.02 mg/kg of normal saline were administered intravenously in the first (atropine) and second (control) groups, respectively. Patients’ mean arterial blood pressure (MAP), heart rate (HR), and incidence of complications after surgery were recorded. Results: The results of the present study revealed that the two groups had no significant difference in terms of the mean MAP in any of the studied times (P-value > 0.05). However, the mean HR during surgery was significantly higher in the atropine group compared with the control group (P-value < 0.05). In addition, although the incidence and severity of delirium in the atropine group were insignificantly higher than that of the control group, respiratory complications such as decreased saturation and laryngospasm were lower in the atropine group compared with the control group (P-value > 0.05). The volume of suctioned secretions in the atropine group, with a mean of 6.26 ± 2.47 cc was significantly lower than its volume in the control group, with a mean of 14.57 ± 4.27 cc (P-value < 0.001). Conclusions: According to the findings of the present study, although the administration of atropine slightly increased delirium incidence, it could reduce the incidence of respiratory complications and secretions after surgery. 摘要

背景:鉴于缺乏研究阿托品对儿童麻醉后护理病房(PACU)谵妄发生率的影响,本研究旨在探讨阿托品对儿童手术后谵妄发生率及其他并发症的影响。方法:本双盲随机临床试验对70例1 ~ 6岁的下腹部手术候选者进行研究。患者被分为两组,每组35名患者。麻醉诱导前,第一组(阿托品)静脉滴注0.02 mg/kg阿托品,第二组(对照组)静脉滴注0.02 mg/kg生理盐水。记录患者平均动脉血压(MAP)、心率(HR)及术后并发症发生率。结果:本研究结果显示,两组在任何研究时间的平均MAP均无显著差异(p值> 0.05)。但阿托品组术中平均HR明显高于对照组(p值< 0.05)。此外,虽然阿托品组谵妄的发生率和严重程度均低于对照组,但呼吸系统并发症如饱和度降低、喉痉挛等均低于对照组(p值> 0.05)。阿托品组抽吸分泌物体积(平均6.26±2.47 cc)显著低于对照组(平均14.57±4.27 cc) (p值< 0.001)。结论:根据本研究结果,阿托品虽能轻微增加谵妄发生率,但可降低术后呼吸道并发症及分泌物的发生率。然而,要在这方面作出更明确的结论,还需要进行更多的研究。
