Radiographic examination revealed a wide-open apex in the palatal canal of the upper left first maxillary molar and there was a periapical radiolucency associated with the tooth. The patient had a history of cyst enucleation surgery done in the same region 3 years ago. The patient had undergone multiple visits in between the final apexification procedure due to pain and a weeping canal.
