Assurance-qualité du service biomédical: application à un hôpital d'outre-mer

Due to the laws regarding hospitalization reform, health institutions are compelled to evaluate their structure, procedures and results. To attain this effort, biomedical departments are required to implement a quality management system, such as the ISO 9002 standard. This standard is becoming the model of choice for a growing number of French biomedical departments. However, as its application requires an effort of interpretation according to departmental structure, the question is: ‘Is the implementation methodology of the ISO 9000 frameworks really accessible without significant adaptations to any institution?’. In initiating this implementation in an over-seas hospital, we extract elements which are intimately linked with the hospital structure and whose study is necessary to the perenniallity of the quality approach: the importance of the equipment set whose maintenance is taken over inward, the means to provide, the evaluation of the local providers and the adaptation of the management modes in the socio-professionnal context.

Du feit des lois portant réforme hositaliere, les établissements de santé se doivent d'intégrer une démarche qualité Afin d'accompagner l'effort demande à l'établissement, le service biomédical est amené à adopter un système qualité formalisé. La réfé rence en termes de démarche qualité des services biomédicaux est l'application des normes internationales ISO 9000, réalisée avec succès dans les départements biomédicaux britanniques et quelques-uns de leurs homologues français. En se souvenant que ces normes sont des règles générales destinées initialement au monde industriel, leur application à un service public ne s'effectue pas sans un effort d'interprétation. Cet effort est juge d'autant plus rédhibitoire qu'il variera selon la structure considérée. En débutant une méthodologie d'application de ces normes dans le contexte contraignant d'un service biomédical outre-mer, nous avons extrait des prêrequis, intimement liés à la structure hospitalière et dont l'étude est nécessaire à la pérennité d'une démarche qualité les moyens à mettre en œuvre, le taux de couverture en interne de la maintenance, l'évaluation des fournisseurs locaux, l'adaptation des modes de management au contexte socioprofessionnel.
