We performed a combined approach -transmastoid plus minicraniotomy- to repair the skull base defect. A multilayer reconstruction of the defect with septal cartilage and temporal fascia was performed. 简介:报告一例复杂的乳突管壁下切除术后巨大脑膜脑膨出的病例,并描述我们修复颞骨脑膜脑疝的首选方法。病例报告:一名20岁的患者,曾因胆脂瘤行III型鼓室成形术并全听骨重建假体,其临床和影像学特征符合巨大颞膜脑膨出的诊断。我们采用联合入路-经乳突骨加小颅骨切开术-修复颅底缺损。采用间隔软骨和颞筋膜多层重建缺损。经过48个月的随访,患者仍无症状,无组织疝的迹象。结论:经乳突突加小颅骨切开术联合入路是一种安全可行的技术,适用于大且前颅底缺损,手术发病率低,即使在同时伴有活动性慢性中耳炎的情况下,也可以进行安全的多层重建。
