Radiofrequency ablation is considered the first-line treatment, it can be curative in almost 95% of patients. Sometimes ablation therapy can fail when the pathway is close to the epicardium. We hereby report a case of a patient with a left lateral accessory pathway. Several attempts of ablation from the endocardium targeting a clear pathway potential failed.
