Coronary angiography revealed thrombotic occlusion of the distal segment of the left anterior descending artery (LAD) and floated thrombus in LAD mid-segment; stenosis of the left circumflex artery (LCx) close to the first obtuse marginal artery (OM1). \nThe main diagnosis. Acute myocardial infarction with ST-segment elevation of the left ventricle anterior wall (anterior-STEMI). Calcific aortic valve disease, severe aortic valve stenosis, significant mitral and tricuspid valve regurgitation, pulmonary hypertension. Heart failure with reduced LVEF (<40%), NYHA class III symptoms. \nIntervention. The decision was to perform urgent stenting in LAD and balloon angioplasty in LCx. Successful hospital discharge was after 14 days. \nConclusions. Take-away lessons: Elderly patients with acute myocardial infarction can be successfully treated in Ukraine. Abstract

92岁女性患者心肌血运重建成功一例报告
介绍。急性心肌梗死患者应及早行血运重建术。73岁以上的患者发生梗死相关心源性休克的风险更高,这是导致死亡的主要原因。不幸的是,对乌克兰老年人心肌血运重建护理知之甚少。我们提出了一个92岁的妇女与急性胸痛病例报告,这是不透露止痛药。重要的临床发现。冠状动脉造影显示左前降支远段血栓闭塞,左前降支中段漂浮血栓;左旋动脉(LCx)靠近第一钝缘动脉(OM1)狭窄。主要诊断。急性心肌梗死伴左心室前壁st段抬高(前stemi)。主动脉瓣钙化性疾病,主动脉瓣严重狭窄,二尖瓣和三尖瓣明显返流,肺动脉高压。心力衰竭伴LVEF降低(<40%),NYHA III级症状。干预。我们决定在LAD患者行紧急支架置入术,在LCx患者行球囊血管成形术。14天后顺利出院。结论。经验教训:老年急性心肌梗死患者在乌克兰可以成功治疗。年龄和合并症不是早期心肌血运重建的禁忌症。
