LIMITED JOINT MOBILITY OF THE HAND: COULD IT BE THE AGING HAND? 手部关节活动受限:是否与手部老化有关?

Background: The progressive diminution of manual dexterity and functional capacity in the hands is an inherent aspect of the aging process. However, when assessing hand dysfunction and limitations in elderly individuals, it is imperative to exclude alternative etiologies before attributing such manifestations solely to age-related deterioration. Case Report: This report presents the case of a 65-year-old female patient exhibiting skin thickening and restricted joint mobility in both hands. Subsequent comprehensive evaluation led to a diagnosis of diabetic cheiroarthropathy, commonly known as limited joint mobility syndrome. Conclusion: The observed decline in hand function among elderly individuals with diabetes mellitus may be attributable to diabetic cheiroarthropathy. This underscores the importance of considering diabetes-related musculoskeletal disorders in the differential diagnosis of age-associated functional deterioration in the hands.

This article explores the crucial and frequently disregarded domain of men’s health as they age, specifically focusing on the consistently increased rates of mortality among males worldwide. Although men display increased cardiovascular risk factors and higher rates of injury, there persists an inequity in the focus given to men’s health in comparison to women’s health. Given the projected increase in the global population of individuals aged 60 and beyond, it is essential to emphasize healthcare for elderly males. Initially grounded in andrology and urology, the World Health Organization has broadened the definition of men’s health to include overall well-being, encompassing physical, mental, and social aspects. Given the increasing life expectancy and the corresponding increase in age-related conditions, adopting a multidisciplinary approach involving a range of healthcare providers is crucial. The review examines critical elements of health in aging men, including genitourinary infections, cardiovascular diseases, malignancies, stress-coping strategies, and the distinct challenges associated with urological issues and sexual health in elderly males.

Avascular necrosis of the femoral head can traumatic or non-traumatic in origin. Embolism (hemoglobinopathies, dysbaric osteonecrosis), corticosteroid treatment, alcohol usage, pelvic irradiation, and genetic susceptibility are among the causes of this clinical entity. Pathogenesis is influenced by thrombophilia, hypofibrinolysis, and hypoangiogenesis. Coronavirus disease 2019 (COVID-19) infection stands as a cause of non-traumatic osteonecrosis of the femoral head. The condition can be related to the use of corticosteroids throughout the disease course of COVID-19. Yet, there could be other disease-related factors that may contribute to the development of osteonecrosis of the femoral head. The current article aimed to present a case with COVID-19 associated avascular necrosis of the femoral head.

SEVERE HYPOFIBRINOGENEMIA IN A PATIENT WITH GIANT CELL ARTERITIS TREATED WITH TOCILIZUMAB: CASE-BASED REVIEW 用托珠单抗治疗巨细胞动脉炎患者的严重低纤维蛋白原血症:病例回顾

The Giant cell arteritis (GCA) is the most common form of systemic vasculitis in elderly patients. The treatment includes high doses of steroids and interleukin (IL)-6 inhibitor tocilizumab, especially in refractory or relapsing disease or in cases where there is an increased risk of steroid-related adverse events. This report discusses the case of a patient with giant cell arteritis who underwent treatment with tocilizumab for four years. The treatment was successful and resulted in clinical remission. However, four years after starting the therapy, the patient developed spontaneous hematomas on their extremities. After further investigation, it was discovered that the patient had developed thrombocytopenia and hypofibrinogenemia, which required substitution therapy. Malignancy and immune-mediated causes of hypofibrinogenemia and thrombocytopenia were ruled out. The patient experienced an extended period of hypofibrinogenemia that lasted for two months after the last dose of tocilizumab. During this time, the levels of CRP remained very low. This could be because the continued inhibition of IL-6 caused impaired hepatic synthesis of acute phase response proteins, which led to low fibrinogen and CRP levels in serum. The purpose of this case-based review is to emphasize the necessity of regular fibrinogen check-ups in GCA patients treated with tocilizumab.

Sleep disturbances or alterations in sleep patterns are commonly observed in older individuals as they age. Moreover, sleep problems may potentially contribute to the impairment of the healthy aging process, thereby leading to frailty in the geriatric population. The vicious cycle between sleep problems and aging can be broken by implementing sleep hygiene strategies that lead to improved sleep quality. The present hypothesis emphasizes that ensuring proper sleep hygiene can contribute to promoting healthy aging among older individuals.

VASCULAR AGING: PATHOPHYSIOLOGICAL MECHANISMS, CLINICAL IMPLICATIONS, AND PREVENTIVE STRATEGIES 血管老化:病理生理机制、临床意义和预防策略

Vascular aging carries a major function in mortality and morbidity among the elderly due to the increased frequency of cardiovascular events. To develop novel preventive and treatment strategies for early vascular aging, it is crucial to know the pathophysiological changes in the blood vessels during the aging process. In this review, we present the molecular mechanisms of vascular aging, including mitochondrial dysfunction, activation of inflammation, epigenetic alterations, and genomic instability. The importance of various diagnostic methods of vascular aging determination and clinical manifestations are discussed. Finally, current preventive strategies to postpone alterations associated with the aging process in the vasculature by targeting the pathophysiological mechanism of aging are presented.

FIBROMYALGIA IN OLDER ADULTS 老年人的纤维肌痛

Fibromyalgia is a chronic pain condition. Mood disorders, sleep problems, fatigue, and temporomandibular disorders can accompany fibromyalgia. The hallmark of fibromyalgia is chronic generalized pain. Yet, numerous diseases can present with generalized pain in older adults. Careful anamnesis and a detailed physical examination are essential to rule out mimicking conditions. Treatment of fibromyalgia in elderly requires particular attention, as well. Older individuals are prone to experiencing adverse effects of certain drugs due to age-related alterations in pharmacokinetics and pharmacodynamics. The aim of this article was to review fibromyalgia in older adults; including its features, differential diagnosis, and management.
