Dextrose Prolotherapy for Supraspinatus Partial Tear: A Case Report

Case report: A 34-year-old male with a history of chronic right shoulder pain presents with tenderness at the lateral shoulder, positive subacromial impingement tests, painful end range of motion and pain score of 6. Ultrasound shows a partial supraspinatus tear at the bursal side. The patient was treated with 15% dextrose to the supraspinatus tendon intrasubstance 3 times, for 4 weeks. Result: Significant pain improvement after the first treatment (VAS 1), with pain-free full range of motion until 4 weeks after treatment and sonographic evidence of supraspinatus tendon healing. Discussion: Dextrose concentrations higher than 12.5% produce an osmotic gradient which stimulates the accumulation of growth factors and inflammatory cells, which in turn can initiate the wound healing process. In this case we can find that the healing process translates to good clinical outcome by ultrasound imaging. 简介:肩袖撕裂约占肩袖疾病的20%,并伴有严重的肩部疼痛,严重影响日常生活活动。病例报告:34岁男性,有右肩慢性疼痛史,表现为肩外侧压痛,肩峰下撞击试验阳性,运动末端疼痛,疼痛评分6分。超声显示滑囊一侧冈上肌部分撕裂。患者给予冈上肌腱实质内15%葡萄糖治疗3次,共4周。结果:首次治疗后疼痛明显改善(VAS 1),治疗后4周无疼痛全活动,超声显示冈上肌腱愈合。讨论:葡萄糖浓度高于12.5%会产生渗透梯度,刺激生长因子和炎症细胞的积累,从而启动伤口愈合过程。在这种情况下,我们可以发现愈合过程转化为良好的临床结果超声成像。结论:葡萄糖前体疗法可作为冈上肌腱部分撕裂的治疗方法,效果良好。
