This study aimed to analyze the effectiveness of chronic illness prevention program using health belief model (HBM).\nSubjects and Method: This was a retrospective cohort study carried at 4 community health centers in Ngawi, East Java, from April to May 2019. A sample of 200 elderly aged 60-74 years old was selected by fixed exposure sampling. The dependent variable was hypertension prevention. The independent variables were prolanis participation, perceived susceptibility, perceived seriousness, and self efficacy.
