This research method uses Research and Development using the ADDIE research development model (analyze, design, development, implementation, evaluation). The respondents in this study were 21 class IV students at SDN Dawuhan Lor. The data collected is quantitative data and qualitative data which is used to determine validity, practicality and effectiveness. The results of the learning media development research are (1) The QR-Code based Booklet learning media was declared valid and very good for use, this was obtained from media experts getting a score of 96.7% and material experts getting a score of 91%, and the average -The average validity of the media and materials produces a validity score of 94%. (2) QR-Code based Booklet learning media is stated to be practical and very good to use. This practicality was obtained from the teacher's response getting a score of 93% and the student's response getting a score of 92.5%, then calculating the average of the results of the teacher's response and the student's response resulting in a practicality score of 93%. (3) QR-Code based Booklet learning media is said to be an effective and very good media to use. This effectiveness is obtained from the results of evaluation tests, in the limited test the average score was 88 and the CBC was 83%, in the extensive test the average score was 96.4 and the CBC was 93%, then from both it was calculated that the average score was 94 and got classical learning completeness (KBK) 90%. (2) Media media pembelajaran Booklet berbasis QR-Code dinyatakan praktis dan sangat baik untuk digunakan. Kepraktisan ini diperoleh dari respon guru mendapat skor sebesar 93% dan respon siswa mendapat skor sebesar 92,5%, selanjutnya dihitung rata-rata hasil respon guru dan respon siswa menghasilkan skor kepraktisan sebesar 93%. (3) Media pembelajaran Booklet berbasis QR-Code dikatakan sebagai media yang efektif dan sangat baik untuk digunakan. Keefektifan ini diperoleh dari hasil nilai tes evaluasi, pada uji terbatas mendapat skor rata-rata 88 dan KBK 83%, pada uji luas mendapat skor rata-rata 96,4 dan KBK 93%, kemudian dari keduanya dihitung rata-rata mendapat skor 94 dan mendapat ketuntasan belajar klasikal (KBK) 90%.
