Esquizofrenia de inicio tardío: A propósito de un caso

Abstract

The case of a 44-year-old male, who was referred for urgent evaluation by the Psychiatry Department after being seen by Clinical Psychology in the context of the appearance of delirious ideas of one week of evolution after quitting smoking and taking bupropion is presented. In the first consultation, he appears with a slightly disheveled appearance and with a delirious discourse, with the presence of ideas of prejudice and persecution, which we are able to dismantle in the first interview with a good ability to understand this situation and the presence of an illness, where at the same time we work on the adherence to medication and to the consultations of the specialty. There is also the presence of auditory sensoperceptive alterations. He presents mood and moderate baseline anxiety reactive to an experiential situation triggered by the disease and partial awareness of the disease. In view of this situation, cariprazine with withdrawal of bupropion are prescribed. Over the weeks, a good response to treatment and adherence to it is observed. After an adequate differential diagnosis and performance of the necessary complementary tests, the presence of late-onset schizophrenia can be concluded.
