This case reports a 61-year-old male patient with renal pelvic sarcomatoid urothelial carcinoma who developed extensive lymph node metastasis 26 days after posterior laparoscopic radical nephrectomy for renal carcinoma, and the patient died on the 45th postoperative day, with the cause of death being advanced malignancy of the tumor. This case highlights the rapid progression of sarcomatoid urothelial carcinoma, and sarcomatoid variants should be identified as early as possible, with active multidisciplinary adjuvant therapy and closer follow-up when feasible. Abstract

Sarcomatoid urothelial carcinoma of the renal pelvis is an extremely rare malignant tumor with a high risk of recurrence and metastasis and a poor prognosis. This case reports a 61-year-old male patient with renal pelvic sarcomatoid urothelial carcinoma who developed extensive lymph node metastasis 26 days after posterior laparoscopic radical nephrectomy for renal carcinoma, and the patient died on the 45th postoperative day, with the cause of death being advanced malignancy of the tumor. 摘要肾盂尿路上皮肉瘤样癌是一种极为罕见的恶性肿瘤,复发转移风险高,预后差。本病例报告一例61岁男性肾盆腔肉瘤样尿路上皮癌患者,因肾癌行腹腔镜后根治性肾切除术26天后发生广泛淋巴结转移,患者于术后第45天死亡,死因为肿瘤恶性进展。本病例提示尿路上皮肉瘤样癌进展迅速,应尽早发现肉瘤样变异,积极多学科辅助治疗,可行时密切随访。回顾性分析该患者的治疗方案和入院情况,为认识和积极治疗这种罕见且致命的尿路上皮癌提供了经验教训。
