The preoperative sonographic findings of FBC and IDC were retrospectively analyzed and compared. \n \n \nResults \nThere was no significant difference between the two groups in the rates of showing irregular shape, which were 86.7% in FBC group and 88.1% in IDC group, respectively (P>0.05), but the rate of showing crab feet or burrs on the edge of lesions in FBC group was lower than that in IDC group(P<0.05). The occurrence rates of posterior echo enhancement and cystic degeneration in FBC group were 81.7% and 71.7% respectively, which were significantly higher than those in IDC group (38.1% and 16.7%)(P<0.001). In addition, the FBC group showed fewer features such as hyperechoic halo and more features such as hypovascular supply than that of IDC group (all P<0.001). \n \n \nConclusions \nIn terms of ultrasonic features, including irregular shape or even crab feet and burrs, FBC can be easily misdiagnosed as malignant tumors. 乳腺纤维囊性病变的超声特征
目的探讨纤维囊性乳腺改变(FBC)的超声表现特点,提高FBC的超声诊断和鉴别诊断水平。方法选取2014年1月至2019年2月福建医科大学附属协和医院经手术病理证实的55例60个病灶的FBC患者和39例42个病灶的浸润性导管癌(IDC)患者。回顾性分析比较FBC和IDC术前超声表现。结果两组在不规则形状出现率上差异无统计学意义,FBC组不规则形状出现率为86.7%,IDC组不规则形状出现率为88.1% (P<0.05),但FBC组不规则形状出现率低于IDC组(P<0.05)。FBC组后回声增强和囊性变性的发生率分别为81.7%和71.7%,显著高于IDC组(38.1%和16.7%)(P<0.001)。此外,与IDC组相比,FBC组出现的高回声晕等特征较少,血管供应不足等特征较多(P<0.001)。结论FBC的超声表现为不规则形状甚至蟹足、毛刺,极易误诊为恶性肿瘤。但后路回声增强、内部散在小囊肿、血供不足及罕见的高回声晕可能是FBC的特征,可与恶性肿瘤鉴别。关键词:超声检查;纤维囊性乳腺改变;浸润性导管癌
