This study uses a non-experimental (observational) descriptive analysis method. The data used is prescription data for pediatric patients with a diagnosis of acute respiratory infection at the Gunug Jati Cirebon Hospital in 2021. The number of samples that meet the inclusion criteria are 131 prescriptions. The potential for medication errors in the prescribing phase has met the legality principle based on the Minister of Health of the Republic of Indonesia Number 73 of 2016 and the determination of the Medication Error Index (NCC MERP). The results of the patient characteristics based on the highest age were 1-5 years, the highest sex was male. The total potential incidence of medication errors in the prescribing phase is 99.2% with a total of 130 prescriptions, the potential for medication errors in the prescribing phase in the administrative study is 96.9% with a total of 127 prescriptions, in the pharmaceutical study 12.2% with a total of 16 prescriptions and in clinical studies by 35.9% with a total of 47 prescriptions.
