Six researches reporting a total number of 271 patients who underwent HALC or open surgery within restorative proctocolectomy (RPC) or total abdominal colectomy (TAC) were included. There was a significant difference between HALC and open surgery groups in the estimated blood loss (mean difference (MD) = 93.44, 95%CI [51.33, 135.54], <em>p</em> &lt; 0.0001), operative time (MD = − 41.17, 95%CI [− 54.41, − 27.94], <em>p</em> &lt; 0.00001) and length of hospital stay (MD = 1.56, 95%CI [0.24, 2.88], <em>P</em> = 0.02). In the terms of post-operation complications, the incidence of ileus (OR 1.27, 95% CI [0.55, 2.95], <em>P</em> = 0.57) and wound infection (OR 1.21, 95% CI [0.53, 2.79], <em>P</em> = 0.66) were similar between the two groups.
