We present a 74-year-old male who underwent cystectomy for bilateral thigh fistulation years after radical prostatectomy and salvage radiotherapy for prostate cancer. MRI and CT cystogram confirmed fistulation from the anterior bladder tracking to bilateral thigh collections. Formal urinary diversion was performed with cystectomy and ileal conduit.
