Hémangiome capillaire caverneux du septum nasal

Introduction

Les hémangiomes sont des tumeurs vasculaires bénignes rares. Histologiquement, on retient les hémangiomes capillaires, caverneux et mixtes. Nous présentons un exceptionnel hémangiome mixte du septum nasal.

Observation

Une femme âgée de 30 ans a consulté pour une obstruction nasale associée à une épistaxis droite. En TDM, une tumeur comblait le rhinopharynx, elle se rehaussait à l’injection du produit de contraste. Elle était pédiculée sur la partie postéro-supérieure du septum nasal. L’artériographie montrait un blush vasculaire. L’exérèse de la tumeur a été faite par voie combinée para-latéro-nasale et endoscopique. L’histologie révélait un hémangiome mixte.

Discussion

Les hémangiomes capillaires et caverneux sont des tumeurs vasculaires, bénignes et rares, qui intéressent surtout la peau de la tête et du cou et les muqueuses orale et nasale. Aucun hémangiome mixte septal n’a été rapportée dans la littérature. Le traitement est chirurgical et doit assurer l’exérèse complète de la lésion. Le pronostic est bon et le risque de récidive est faible.

Introduction

Hemangioma are rare benign vascular tumors. Histologically, there are several types including capillary, cavernous and mixed hemangioma. We present a very rare mixed hemangioma of the nasal septum.

Case report

A 30-year-old woman consulted for unilateral epistaxis and nasal obstruction. CT scan revealed a tumor filling the rhinopharynx, strongly enhanced after injection of contrast medium. It was pedicled on the posterior and superior nasal septum. Arteriography revealed a vascular blush. Tumor exeresis was performed by paralateronasal approach and endoscopic surgery. The histological examination identified a mixed hemangioma.

Discussion

Capillary and cavernous hemangiomas are rare and benign vascular tumors developing in the skin and subcutaneous tissues of the head and neck, and/or oral and nasal mucosa. No mixed hemangioma exclusively involving the nasal septum had been previously reported. Complete surgical resection is the treatment of choice, providing a favorable prognosis with low risk of recurrence.
