This leads to atypical presentations, delayed diagnosis and poor outcome. Here we present a case of an elderly diabetic patient with chronic kidney disease on maintenance haemodialysis who presented with lower limb pain initially and had no urinary complaints. The evaluation revealed the presence of extensive emphysematous pyelonephritis, cystitis and myositis.
