Some of them are rare pathological processes and are difficult to diagnose and surgical treatment. The mucocele of the appendix is a rare, more often benign neoplasm, developing by widening the lumen of the appendix due to excessive accumulation of mucin. One of the most dangerous complications of a mucocele is a spontaneous or iatrogenic rupture with the development of a peritoneal pseudomyxoma (mucinous carcinomatosis), as a result of the dissemination of goblet cells, which are fixed in abdominal flanks and produce mucin. Article presents a case report of a 32-year-old patient with mucinous cystadenoma of the appendix and developed of peritoneal pseudomyxoma, who underwent laparoscopy with a conversion to a total laparotomy, right hemicolectomy, resection of the greater omentum and terminal ileostomy. Results of immunohistochemistry: highly differentiated neuroendocrine tumor, germinating all tissues of the appendix, spreading on adipose tissue.
