The disease severity was evaluated according to the RANSON score and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score, and severe pancreatitis was used as the outcome index. Univariate logistic regression analysis and receiver operating characteristic (ROC) curve analysis were performed on single indicators that were statistically significant between mild and severe patients to determine the distinguishing value for disease severity. \n \n \nResults \nThe RANSON score was graded: 43 cases of severe patients (RANSON≥3) and 99 cases of mild patients (RANSON<3); the difference in albumin (ALB) levels between the two groups was statistically significant (P<0.05). Univariate logistic regression analysis and ROC curve analysis were performed on ALB (OR=0.88, P<0.001). The area under the ROC curve and its 95% CI were 0.73 (0.63, 0.83). APACHE Ⅱ score classification: 94 cases of severe patients (APACHE Ⅱ score≥8) and 48 cases of mild patients (APACHE Ⅱ score<8). The difference in alanine aminotransferase (ALT) levels in the two groups were statistically significant (P<0.05). Univariate factor logistic regression analysis and ROC curve analysis were performed on ALT (OR=1.001, P=0.314). 摘要

目的探讨肝功能指标在预测急性胰腺炎(AP)患者病情严重程度中的临床价值,为临床治疗提供参考。方法对142例急性胰腺炎患者进行回顾性研究。根据RANSON评分和急性生理与慢性健康评估Ⅱ(APACHEⅡ)评分评估疾病严重程度,以重症胰腺炎作为结局指标。对轻、重度患者间有统计学意义的单项指标进行单因素logistic回归分析和受试者工作特征(ROC)曲线分析,确定疾病严重程度的区分值。结果RANSON评分分级:重度患者43例(RANSON≥3),轻度患者99例(RANSON<3);两组患者白蛋白(ALB)水平差异有统计学意义(P<0.05)。对ALB进行单因素logistic回归分析和ROC曲线分析(OR=0.88, P<0.001)。ROC曲线下面积及其95% CI为0.73(0.63,0.83)。APACHEⅡ评分分类:重度患者94例(APACHEⅡ评分≥8),轻度患者48例(APACHEⅡ评分<8)。两组患者丙氨酸转氨酶(ALT)水平差异均有统计学意义(P<0.05)。对ALT进行单因素logistic回归分析和ROC曲线分析(OR=1.001, P=0.314)。ROC曲线下面积及其95% CI为0.61(0.50,0.71)。结论ALB和ALT是预测AP严重程度有价值的生物标志物。关键词:肝功能检测;急性胰腺炎;疾病的严重程度
