Case report : A 41-year old woman presented to an ophthalmology clinic complaining of near vision disturbance. On examination, there was dilatation and telangiectasis of the conjunctival vessels on the temporal side of the right eye associated with right hemicranial and neck cutaneous involuted hemangioma ipsilateral to the conjunctival lesion. The patient experienced vaso-vagal syncope for approximately 15 minutes after mydriatic eye drops, 2 months prior to presentation. A magnetic resonance angiogram (MRA) showed right hypoplasia of the lateral and sigmoideus venous sinus ipsilateral to the external right hemangioma with predominantly compensatory drainage at the contralateral left system. At the neck vessels, the MRA showed dominant drainage through the lateral and jugular left sinus by the hypoplasia of the contralateral right system. Conclusion: This case shows the association of telangiectasis of the conjunctiva, and cutaneous hemangiomas of the head and neck with anomalies involving the central nervous system (CNS) as described by Pascual-Castroviejo in 1978, which they called cutaneous hemangioma– vascular complex syndrome. 摘要

目的:报告一例伴有皮肤血管瘤和颅内血管异常的患者,在滴水杨液后出现血管迷走神经性晕厥(VVS)。病例报告:一名41岁女性到眼科就诊,主诉近视力障碍。检查发现,右眼颞侧结膜血管扩张和毛细血管扩张,伴有与结膜病变同侧的右半颅和颈部皮肤受累血管瘤。患者在就诊前2个月,滴重液后出现血管迷走神经性晕厥约15分钟。磁共振血管造影(MRA)显示右侧外侧和乙状静脉窦发育不全,与右侧外血管瘤同侧,对侧左侧系统主要代偿性引流。在颈部血管处,MRA显示对侧右系统发育不全导致左侧外侧和颈静脉窦引流为主。结论:本病例显示结膜毛细血管扩张和头颈部皮肤血管瘤与中枢神经系统(CNS)异常的关联,Pascual-Castroviejo在1978年描述过,他们称之为皮肤血管瘤-血管复杂综合征。患者在滴入无泪滴眼液后出现VVS后作出诊断。
