Since position and centration have shown to affect the effectiveness of multifocal intraocular lenses (MFIOLs), some authors have proposed using the "coaxially sighted IOL light reflex" as a landmark to allow consistent centration of MFIOLs. Postoperatively, assessing the location of the intraocular lens with respect to the visual axis is also important. Centration of the MFIOLs can be identified postoperatively by slit lamp biomicroscopy, but it is still a qualitative method. The Nidek OPD Scan III is used. The instrument obtains retro illumination images, which allows the observation of the diffractive rings of the multifocal intraocular lenses implanted following phacoemulsification. The retro illumination image describes the location of the visual axis (white cross with blue borders +), the center of the photopic (pink cross) and mesopic (turquoise cross) pupil, quantifies the distance between the center of the pupil and the visual axis, and places the visual axis in degrees. It is attempted to match the rings of the lens with that of the acetate grid. Once the best possible equidistance between the lens and grid rings has been achieved. This method can be useful to be able to have reference of a real value of decentration of the lenses and thus have a surgical plan and try to center the lenses. We are also creating a technique to properly center the lenses, in addition to performing an analysis of the results in terms of postoperative visual quality in relation to the lens's decentration and thus see how this really affects.

Purpose: To determine the activity of metformin in an in vitro model of posterior capsule opacification (PCO).Design: Experimental laboratory research. Methods: The HLE-B3 lens epithelial cell line was treated with PCO induction media (PCOM) supplemented with transforming growth factor-beta (TGF-β) and fibroblast growth factor (FGF). Different metformin concentrations (0-100 mM) were used. The following cellular parameters were assessed: (1) survival, using a viability assay; (2) morphology, via microscopy and image analysis; (3) migration, using the wound assay; (4) and expression of epithelial (Pax6, E-cadherin) and mesenchymal (α-smooth muscle actin or α-SMA, fibronectin) markers via Western blot. Expression of the uptake receptor SLC22A1 was evaluated in HLE-B3 and in human donor eyes with Western blot and immunohistochemistry, respectively. Statistical analysis of variance (ANOVA) with Tukey post-hoc test was done for analysis of cytotoxicity, morphology and migration data. Results: Metformin was lethal to half (LC50) of the cells at 30 mM, and a decrease in viability (P<0.05) was noted at 5 mM. LECs in PCOM treated with 1 mM metformin showed increased Pax6 and E-cadherin and decreased α-SMA and fibronectin expression. LECs in PCOM treated with metformin also maintained epithelial morphology. Migration was inhibited with 0.5 mM metformin (P<0.05). Both HLE-B3 and the lens epithelium in donor eyes were found to express SLC22A1.Conclusion: Metformin decreased survival and migration in LECs, maintaining epithelial phenotype and reducing mesenchymal marker expression. We report a case of a child with nephrotic syndrome (NS), who presented an acute convergent strabismus, double vision and papilledema one week after a history of badly treated sinusitis. The image study revealed an extensive thrombosis of the intracranial sinuses that was treated with intravenous heparin and oral corticosteroids. Four months later, the case evolved with spontaneous resolution of the ocular deviation and no diplopia.

Purpose: To describe a case of inferior bullous keratopathy caused by iris fibers resolved with laser diode 532.Case report: A 74-year-old woman consulted for deterioration of visual acuity in right eye, accompanied by foreign body sensation. A phacoemulsification surgery with IOL implant had been performed in both eyes three months before. Best corrected visual acuity reached 20/40 in right eye. Slit lamp examination revealed bullous keratopathy located in the central inferior quarter of the cornea of her right eye. Gonosioscopy was performed to rule out having nucleus fragments in the inferior angle. With higher magnification, a pair of fine undulating fibers were observed coming from the anterior layers of the iris and making contact with the posterior face of the cornea. Photocoagulation of the iris fibers was performed with laser diode 532 and photodisruption with Nd-YAG laser, applied at the base of the fibers. After nine days, resolution of corneal edema, and disappearance of bullae was observed. Best corrected visual acuity improved to of 20/25.Conclusions: photocoagulation of iris fibers with laser diode 532 complemented with photodisruption with Nd-YAG laser, in patients with iridoschisis, is an alternative to avoid endothelial decompensation and corneal edema, which may require a corneal transplantation.

RESUMEN: Objetivo : Describir resultados y complicaciones de pacientes llevados a cirugia de catarata por facoemulsificacion mas implante de lente intraocular en el Hospital de San Jose y explorar la asociacion entre posibles factores relacionados con complicaciones intraoperatorias. Materiales y metodos: Cohorte retrospectiva. Se incluyeron todos los ojos operados de catarata por facoemulsificacion entre 2010 y 2016. Se excluyeron casos con registros insuficientes, convertidos a extraccion extracapsular de catarata por causas diferentes a complicaciones y en los cuales se realizo cirugia combinada programada. Se describieron caracteristicas especiales intraoperatorias del ojo, dureza de la catarata, colaboracion intraoperatoria del paciente, resultados visuales y refractivos y complicaciones, durante el primer mes postoperatorio. Se evaluo la relacion entre caracteristicas especiales del ojo y complicaciones intraoperatorias usando la prueba de chicuadrado y considerando un p valor inferior a 0.05 como estadisticamente significativo Resultados: Se incluyeron 2175 ojos, se excluyeron 128, para 2047 estudiados. Al mes postoperatorio, el 86.8% alcanzo agudeza visual corregida de 20/40 o mejor y el 75.2% equivalente esferico de ± 1.00D. Los ojos con alguna caracteristica especial presentaron mas complicaciones intraoperatorias (p=0.01), al igual que aquellos con catarata extremadamente dura (p=0.003) y con colaboracion deficiente durante la cirugia (p<0.000). La complicacion intraoperatoria mas frecuente fue ruptura de capsula posterior y la postoperatoria, opacidad de capsula posterior. Conclusiones: Se obtuvieron resultados visuales y refractivos satisfactorios y la incidencia de complicaciones fue similar a la reportada en la literatura. La identificacion de caracteristicas especiales intraoperatorias es importante dada su asociacion con mayor incidencia de complicaciones. Los ojos con alguna caracteristica especial presentaron mas complicaciones intraoperatorias (p=0.01), al igual que aquellos con catarata extremadamente dura (p=0.003) y con colaboracion deficiente durante la cirugia (p<0.000). La complicacion intraoperatoria mas frecuente fue ruptura de capsula posterior y la postoperatoria, opacidad de capsula posterior. Conclusiones: Se obtuvieron resultados visuales y refractivos satisfactorios y la incidencia de complicaciones fue similar a la reportada en la literatura. La identificacion de caracteristicas especiales intraoperatorias es importante dada su asociacion con mayor incidencia de complicaciones. ABSTRACT: Purpose: To describe outcomes and complications of patients underwent phacoemulsification cataract surgery with intraocular lens implantation in the San Jose Hospital and explore the association between possible factors with intraoperative complications. Materials and methods: Retrospective cohort. There was included all the eyes undergoing phacoemulsification cataract surgery between 2010 and 2016. There was excluded cases with insufficient records, cases converted to extracapsular cataract extraction by causes different of complications and those wich underwent scheduled combined surgery. There was described intraoperative special characteristics of the eye, cataract hardness, intraoperative colaboration of the patient, visual and refractive outcomes and complications during the first postoperative month. The association between special characteristics of the eye and intraoperative complications was evaluated using the chi square test considering a p value less than 0.05 as statistically significant.
