Allergie aux hymenoptères

Abstract

Les insectes piqueurs jouent un rôle très important et même parfois préoccupant dans le domaine de l'allergie clinique.

Ce travail porte sur le problème de l'identification des principaux hyménoptères (abeilles, guêpes, frelons) ; la composition et la toxicité des venins d'hyménoptères ; l'aspect immunologique de l'allergie aux hyménoptères, avec rappel des différentes techniques utilisées, l'étude des antigènes et la recherche des anticorps chez les malades sensibilisés.

Les auteurs rapportent leur expérience pédiatrique parisienne qui porte sur 15 cas d'enfants allergiques aux piqûres d'hyménoptères, âgés de 2 à 17 ans : aucun cas mortel constaté, mais plusieurs cas graves.

Enfin, les auteurs exposent leur opinion sur la conduite à tenir en urgence devant une réaction allergique consécutive à une piqûre d'hyménoptère, et surtout la nécessité d'un traitement préventif par désensibilisation spécifique avec des allergènes aqueux on semi-retards, dont les résultats d'ensemble sont très satisfaisants.

Stinging insects play an important and often disturbing role in clinical allergies.

This research is concerned with the problem of identifying the main Hymenoptera (bees, wasps, hornets) ; the composition and toxicity of Hymenoptera venom ; the immunological aspect of allergy to Hymenoptera with a reminder of the various techniques used, an analysis of the antigens and detection of anti-bodies in sensitized patients.

The authors describe their paediatric experience in Paris involving 15 cases of children, between the ages of 2 and 17, allergic to Hymenoptera stings : no fatal cases were found, but several serious ones.

Finally, the authors give their views on emergency treatment for allergic reaction following a sting by one of the Hymenoptera, and stress the need for a preventive treatment by specific desensitization with aqueous or semi-delayed-action allergens, the results of which are, on the whole, very satisfactory.
