Abstract

超声引导下儿童感染性休克液体管理的临床研究
目的探讨重症监护医师进行重症监护胸部超声检查(CCUE)对儿科重症监护室(PICU)感染性休克患者体液管理的影响。方法选择深圳市宝安区妇幼保健院PICU确诊为感染性休克的40例患儿。2017年1月至2018年12月在PICU住院的22人,由4名具有中国危重超声研究组(CCUSG)证书的PICU医生护理,被定义为CCUE组。2014年1月至2015年12月,18名无法使用CCUE的PICU患者被招募为对照组。两组均根据感染性休克管理指南进行常规抗休克和抗感染治疗,并进行机械通气。对照组按照常规方案进行液体管理。而在CCUE组中,CCUE用于监测血流动力学状态,以便在液体管理中进行调整。结果与对照组比较,CCUE组机械通气时间短,入院后48小时内液体进出量少[(4.68±2.06)d vs.(7.33±0.49)d,(6.34±1.85)ml/(kg·h)vs.(8.55±0.39)ml/PICU中的电击。关键词:败血症休克;流体管理;急诊会诊中的危重症胸部超声检查
