Objective To investigate the characteristics of sublingual microcirculation in children with sepsis, and to explore the clinical value of sublingual microcirculation in the early diagnosis, therapeutic effect evaluation of sepsis in children. Methods Children with sepsis and community acquired pneumonia(CAP) admitted in our hospital from December 2018 to December 2019 were included in the study, which was divided into sepsis group and control group(CAP group). In the sepsis group, large circulation indexes and sublingual microcirculation indexes were collected at 0, 6, and 24 hours after admission.Sublingual microcirculation indexes were collected by bypass dark field imaging.In the control group, large circulation indexes were collected after admission, and sublingual microcirculation indexes were collected during tracheal endoscopic examination after sedation.De Backer score(DBs), hetergeneity index(HI), hetergeneity index small(HIs), proportion of pefusedvessels(PPV), proportion of pefused small vessels(PPVs), total of vessel density(TVD), pefused vessel density(PVD), pefused small vessel density(PVDs)were selected as the evaluation index of sublingual microcirculation.The changes of macrocirculation and microcirculation indexes in the two groups were compared, and the correlation between age and microcirculation indexes in the control group was analyzed. Results A total of 71 children, including 10 sepsis cases and 61 CAP cases, were collected.Among the 61 children with CAP, 9 children (2-48 months) in the same age range as the sepsis group were divided into control group A, and the indicators between the two groups were analyzed.Compared with control group A, PPV and PPVs were lower and DBs, HI, HIs, TVD were higher in the sepsis group at 0 hours(P 0.05). At 24 hours, HI, PPV and PPVs improved, no difference was found compared with the control group A(P 0.05). The age of the control group had a moderate negative correlation with HI, and a weak negative correlation with HIs(r=-0.420, P=0.001; r=-0.387, P=0.002). Conclusion HI, HIs, PPV and PPVs are sensitive indexes of microcirculation disorder in children with sepsis.Fluid resuscitation therapy can improve the indexes of sublingual microcirculation in children with sepsis, but the improvement of sublingual microcirculation in children with sepsis is later than the recovery of macro circulation.HI and HIs are negatively correlated with month age in children with CAP. In recent years, the incidence of deep vein thrombosis(DVT) in children is on the rise.If the embolus falls off, it will cause complications such as pulmonary embolism, which may cause disability and death.Therefore, early prevention and timely treatment of DVT are important to improve survival rate.Due to the complexity and particularity of children′s conditions and the lack of large-scale trials and statistics, there is no uniform standards at home and abroad.This article combined some current clinical advances to provide a reference for the prevention, diagnosis and treatment of DVT. In pediatrics, one of the most common diseases is infectious diseases, which is caused by pathogens invading the body and accompanied by local tissue damage or systemic inflammatory reactions.Children′s undeveloped immunity and the consequent rapid progress of the disease urge early diagnosis and timely treatment.However, there is no significant specificity for the clinical symptoms of various infectious diseases, especially in the early stage.Pathogen detection, as a diagnostic gold standard, is not beneficial for the early diagnosis due to the strict requirements for the test environment, long time consumption, low positive rate, and easy contamination.Compared to other methods, blood routine is the most basic and inexpensive test item in clinical practice with the advantage of simple operation and short time.It mainly detects the quality, quantity, and morphological changes in blood components, such as white blood cells, red blood cells, and platelets.It is a valuable test for early diagnosis, differential diagnosis, and evaluation of the severity as well as the treatment efficacy for infectious diseases.Therefore, blood routine test currently is the most preferred method for clinical diagnosis of infectious diseases in pediatrics. Infectious diseases have a rapid onset and progress.Because of the complicated pathogenic microorganisms, the diagnosis and treatment of infectious diseases are difficult and life threatening.Early and accurate identification of pathogenic microorganisms is important for infection determination, which is helpful to improve prognosis.Now, there are various techniques for the pathogenic microorganisms test, but how to correctly utilize and analyze the test results is still a difficult problem in the clinic.It is important to note that the infection is a clinical diagnosis, and all determinations must be based on the clinic.The article summarized the role of pathogenic microorganism tests in clinical infection determination, in order to help clinical doctors to improve their understandings. Severe infection and sepsis are the main causes of mortality in pediatric intensive care units(PICU). Severe infection in critically illness is characterized by acute onset, rapid progression, associated with multiple organs dysfunction, and several pathogens involved.It is critical to distinguish the main pathogen as soon as possible for clinical decision-making.Up to date, precision diagnosis and individualized strategy attract more attention in the field of critical care medicine.The improvements of molecular biology, the combination of multi-omics, and metagenomics sequencing, etc may yield many new insights into precision diagnosis of etiology.Inflammatory response biomarkers are defined as conventional inflammatory factor, acute phase protein(APP), blood RNA biosignature, metabolic molecules, immune cell markers and so on, which are promising indicators for identification of pathogenic microorganisms in severe infection. Objective Based on a cross-sectional survey about the status of analgesia and sedation nursing behavior of PICU nurses in Northeast China and Inner Mongolia, to analyze the influencing factors and to provide reference for further constructing standardized training programs and developing standardized for analgesia and sedation nursing process. Methods The self-designed questionnaire was made for investigation from 435 PICU nurses in 18 hospitals in Northeast China and Inner Mongolia. Results The average questionnaire score of the analgesia and sedation nursing behavior of PICU nurses was(70.54±13.63)in Northeast China and Inner Mongolia.The behavioral scores were different in each group between different age, nursing age, PICU nursing age, educational background, professional title, and whether was the intensive care unit nurse, whether trained in analgesia and sedation, whether have experience in caring children with analgesia and sedation, and the differences were all statistically significant(P<0.05). The main factors influencing the analgesia and sedation behavior of PICU nurses were whether have received training in analgesia and sedation, whether have experience in caring children with analgesia and sedation treatment and their attitude score to analgesia and sedation. Conclusion There is still much space for development in analgesia and sedation nursing behavior of PICU nurses in Northeast China and Inner Mongolia.The more analgesia and sedation training PICU nurses take participate in, the more experience and positive attitudes they have in nursing children with analgesia and sedation, their clinical nursing practice ability could become more stronger. Early accurate identification of bacterial and viral infections in acute infections has been a clinical challenge.With the rapid development and progress of various clinical tests and detection technologies, more and more indicators of etiology, human genetics and molecular biology are used to identify bacterial and viral infections.In the future, more test indicators will be adopted point-of-care testing method to facilitate clinical application.There will be a more accurate and abundant biomedical information platform and a more efficient hospital information system for comprehensive analysis of multiple indicators to provide more accurate pathogen diagnosis and contribute to the best clinical practice. Infection is a major childhood disease, which is a documented or suspected systemic or local infection caused by any pathogen.Most children are mildly infected with a good prognosis, while a small part progress quickly, which causes multiple organ dysfunction, even leads to death.Infection is one of the main causes of death in children.Fever is one of the most common symptom in infection, it′s controversial to predict the severity of illness by using the fever degree.Studies pointed out that children′s symptoms of the corresponding site of infection are better for predicting the severity of infection.So it′s important to identify the symptoms of infection and intervene.Herein, we reviewed the common symptoms and signs of infection in childhood. Human adenovirus is one of the most important pathogen causing acute respiratory infections in infants and young children, and is easy to cause severe adenovirus pneumonia and respiratory distress syndrome, which can cause death and varying degrees of sequelae.The pathogenesis of human adenovirus-induced severe adenovirus pneumonia remains unclear.It is currently believed that the main predisposing &factors for the occurrence and development of adenovirus pneumonia are immune dysfunction and immune dysfunction after viral infection.This article reviewed the research progress on the pathogenesis of adenovirus pneumonia induced by human adenovirus. Infectious disease is common in children.Accurate and rapid pathogen diagnosis can help clinicians to make targeted treatment and improve the outcome of children.The next-generation sequencing has been developed and applied in clinic recently, which has become a new method of pathogen diagnosis.Metagenomicnext-generation sequencing is the most widely used for pathogen detectation in infectious disease.It can detect all pathogens nucleic acids of the samples unbiasedly.This technology includes sample collection, nucleic acid extraction, library preparation, high-throughput sequencing, biological information analysis and report interpretation.As the complication of whole process involving clinical, microbiological and bioinformation fields, the interpretation of the report results needs to be carefully considered and verified by clinicians depends on the situation of patients and other auxiliary diagnostic methods.Clinicians need to understand next-generation sequencing in order to make better use of this technology and serve patients.
