Abdominal radiographs showed severe pneumoperitoneum and an intestinal mass. Computed tomography revealed massive pneumoperitoneum associated with intestinal perforation, with cervical and thoracoabdominal wall emphysema, pneumomediastinum, pneumoretroperitoneum, pneumoscrotum, and peritonitis. Surgery confirmed a ruptured jejunal mass, and histopathologic evaluation of the excised tissues revealed intestinal lymphoma. Clinicians should therefore consider the possibility of gastrointestinal perforation in patients with severe pneumoperitoneum even where there is no history of trauma.
