Neoadjuvant therapy is a useful adjunct in large and recurrent tumors. Molecular targeting with imatinib mesylate is a promising option. Optimum neoadjuvant therapy followed by Moh’s micrographic surgery is the best therapeutic modality with least morbidity and mortality. Dermatofibrosarcoma protuberans is a slow growing deceptive tumor. It is a low grade malignancy with potential to develop local recurrence as well as metastases especially to the lungs.
