The most common applications in ophthalmology include correction of posterior capsular opacification by capsulotomy and treatment of angle closure glaucoma by peripheral iridotomy. We present the case of a 62-year-old pseudophakic male with symptoms of glare and decreased vision in his right eye whose ocular examination revealed four focal areas of posterior synechiae. Synechiae were refractory to subsequent mydriatic therapy, and Nd:YAG laser was successfully used to achieve posterior synechiolysis.
