The Organ of Vision Lesions in Some Hemoblastoses

Abstract

В статье рассматриваются глазные проявления при гемобластозах. Представлены подробное описание повреждения внутриглазных и внеглазных структур, включая поражение орбиты и придаточного аппарата глаза, а также нейроофтальмологические изменения при наиболее распространенных формах лейкоза и лимфомах. Рассмотрены их первичные клинические проявления, частота их выявления, а также механизмы развития, которые связаны как с прямой инфильтрацией различных структур глаза, так и со вторичными нарушениями системы гемостаза, анемией и тромбоцитопенией. Особое внимание уделяется маскарадному синдрому – характерному признаку опухолевого процесса в глазу. Также оценены влияние терапии на орган зрения, изменения, развивающиеся на фоне иммунодефицита в результате токсического воздействия полихимиотерапии, таргетной и лучевой терапии, а также побочные эффекты аллогенной трансплантации гемопоэтических стволовых клеток, в частности развитие реакции «трансплантат против хозяина» в глазу реципиента. The article discusses ocular manifestations in hemoblastosis. A detailed description of the damage to intraocular and non-ocular structures, including damage to the orbit and appendage of the eye, as well as neuro-ophthalmological changes in the most common forms of leukemia and lymphoma, is presented. Their primary clinical manifestations, their frequency of detection, as well as the mechanisms of development associated with both direct infiltration of various structures of the eye and secondary disorders of the hemostasis system, anemia and thrombocytopenia are considered. Special attention is paid to the masquerade syndrome – a characteristic feature of the neoplastic process in the eye. The effect of the therapy on the organ of vision, changes developing against the background of immunodeficiency as a result of the toxic effects of polychemotherapy, targeted and radiation therapy, as well as side effects of allogeneic hematopoietic stem cell transplantation, in particular, the development of a graft-versus-host reaction in the recipient’s eye, are also considered.
