The pancreas from a surgical perspective: an illustrated overview 从外科角度看胰腺:图解概述

: The human pancreas is a solitary retroperitoneal organ which can be found in the pararenal space. It receives its blood supply from both embryologic foregut and embryologic midgut sources and has both endocrine and exocrine functions. Its chief endocrine function is the regulation of serum glucose levels and its principal exocrine function is the production of digestive enzymes. The position of the pancreas and its anatomical relations make pancreatic surgery challenging and high-risk. This article aims to summarise the key aspects of the pancreas which are relevant to the surgeon/surgical trainee in a concise and easy-to-follow format. Firstly, we cover the anatomy of the pancreas including its relations, blood supply, innervation and ductal anatomy. We have focused on surgically relevant anatomy so that this article can help to refresh an individual’s knowledge prior to assisting in theatre. Secondly, we describe anatomical variants and their clinical implications, along with further illustrations. We hope that readers will feel better prepared to manage patients with these variants having read this article. Thirdly, we discuss the exocrine and endocrine functions of the pancreas in a concise format whilst also discussing the tumours which can arise from these groups of cells. We describe how these tumours commonly present and how they are typically managed. Finally, we include a short section on other surgically-relevant conditions which can affect the pancreas.

Staying safe with laparoscopic cholecystectomy: the use of landmarking and intraoperative time-outs 腹腔镜胆囊切除术的安全性:使用标记和术中暂停

Laparoscopic cholecystectomy is one of the most common procedures in General Surgery. Devastating complications of laparoscopic cholecystectomy include vascular and bile duct injury. Bile duct injury is an especially serious and potentially life-threatening complication that occurs at a reported rate of 0.08–1.5% (1–4). Vasculobiliary injuries can be prevented by following an algorithmic approach to laparoscopic cholecystectomy. At our institution, we have devised a culture for safe laparoscopic cholecystectomy designed around two principles: landmarking and intraoperative time-outs to determine if it is safe to progress. This standardized approach and decision tree have been helpful in promoting safe completion of laparoscopic cholecystectomies even in the most challenging cases (5). We have disseminated this approach to safe laparoscopic cholecystectomy to several divisions of General Surgery in our region through educational rounds. The purpose of this manuscript is to describe our clinical pathway and highlight the art we have developed to illustrate the algorithmic approach. These educational materials may be valuable tools for the broader community of General Surgeons to promote a culture of safety for laparoscopic cholecystectomies.

A simple method to animate suture techniques 一个简单的方法动画缝合技术

A variety of visual resources exist to illustrate suture techniques in dermatologic surgery, but most of these resources consist of static graphics and images. While a series of illustrations may be helpful to conceptualize techniques, the true motions and movements that are essential to correctly implementing them are not always adequately represented. Animations may solve this problem, but they are often made by animators with no medical or surgical experience and therefore may lack the subtleties to correctly understand the technique. We present a method that is both simple and efficient to create a variety of suture animations, thereby allowing surgeons, fellows, residents, and students with minimal to no animation experience to easily animate desired techniques for learning purposes. While these methods may be expanded to other areas of dermatologic surgery, we focus only on suturing techniques.

We highlight the common anatomical landmarks of the upper gastrointestinal tract ( Table 1, Figure 2 ) and endoscopic techniques for successful esophageal intubation, gastric retroflexion, duodenal access and tissue sampling using biopsy forceps.

This is a brief introduction to the components and proper handling of a laparoscope for medical students, so that they can learn how to best aid their surgical teams and patients during laparoscopic cases.
