The Correlation between Clinical and Pathological Lymph Node Status in Oral Squamous Cell Carcinoma 口腔鳞状细胞癌临床与病理淋巴结状态的关系

Introduction: The presence of nodal metastasis from OSCC is the single most important prognostic feature. Extranodal extension further decreases survival by approximately half. Clinical evaluation of the nodal basin has been found to correlate poorly with pathological nodal status. Objectives: Determine the congruency between clinical and pathological nodal status and the incidence of extranodal extension (ENE). Describe the frequency distribution of age, sex, smoking habits, anatomical location, and histology in a South African population. Methods: Ninety-one consecutive cases of oral squamous cell carcinoma were retrospectively studied and the clinical (cN) and pathological (pN) TNM nodal staging compared using the 8th edition of the AJCC/UICC TNM staging system. Extranodal extension was determined in all patients with OSCC, as well as in the subgroup with clinically nodal negative (cN0) disease. The socio-demographical information was described. Results: The cN and pN were congruent in 41.7% (n = 38/91), upstaged in 35.2% (n = 32/91), and down staged in 23.1% (n = 21/91) of cases. Twenty-two patients (24.2%) had pathological evidence of ENE. Males and females constituted 67% and 33% of the study sample, respectively. The median age at the time of diagnosis was 57.3 years (range 5-85 years). A smoking history was present in 73.6%, absent in 14.3%, and unknown in 12.1% of patients. The tongue (38.4%) and floor of mouth (29.7%) were the most common anatomical subsites for primary tumours. Most tumours were conventional squamous cell carcinomas with poorly differentiated grade (62.6%) Conclusion: There is poor correlation between clinical and pathological nodal status when the 8th TNM staging is used. ENE at the time of diagnosis is common among patients with OSCC.

Influence of Acetaldehyde on Oral Epithelial Cells 乙醛对口腔上皮细胞的影响

Acetaldehyde, a product of alcohol metabolism, has been recognized a risk factor for carcinogenesis. To clarify the carcinogenic effects of acetaldehyde in the oral cavity and mechanisms involved, normal oral epithelial cells were incubated with several concentrations of acetaldehyde and assays of genetic and epigenetic changes were performed. Acetaldehyde induced malignant transformation of normal oral epithelial cells and demethylation of the erbB2 gene. In addition, regulation of DNA methylation was suppressed when cells were subjected to alternating exposure to medium containing acetaldehyde and medium without acetaldehyde. These findings suggest that habitual alcohol drinking increases the risk of developing oral cancer, and that an appropriate abstinence period may reduce this risk.

Head and neck cancer (HNC) is the seventh most common cancer worldwide [1], with 887,659 new cases in 2018 [2]. The time trends in HNC rates from 1983 to 2002 vary by geography [3]. The 2018 age-standardized rate of oral cavity varies from 1.2 per 100,000 in Western Africa to 12.9 in Southeast Asia [2]. The major risk factors are tobacco and alcohol for all HNC subsites, except for HPV infection which is likely the major cause of oropharyngeal cancer in developed countries.

Oral cancers are among the 10 most common tumors in Europe and the United States. The major recognized risk factors include smoking and alcohol what is possible explanation for higher incidence among men. The risk of developing a tumor of the oral cavity is 38 times higher in the population that consuming alcohol and cigarettes. Cancer of the oral cavity is a serious disease and only half of the patients survive the next five years. According to the World Health Organization classification, oral cancer includes malignant tumors in the lips area, anterior two thirds tongue, gingiva, bottom of the oral cavity, retromolar triangle, hard palate and mucous membranes of the oral cavity. In spite of the advancement of medicine, оral cancer remains a malignant tumor with a high mortality rate. The reason for this was primarily enhanced by the tumor stage at the time of diagnosis. On the other hand, the oral cancer is a tumor that can be detected by a simple examination of the oral mucosa at an early stage, before it starts to cause symptoms.

However, it is difficult to completely diagnose the N status preoperatively and the development of secondary cervical lymph node metastasis.
