Sodium-glucose cotransporter 2 inhibitors are a class of drugs used for the treatment of patients with type 2 diabetes mellitus and heart failure regardless of the coexistence of diabetes. The potential impact of this treatment in patients with heart failure is so huge that they have been named “the statins of the 21st century”. In some studies they appeared to mildly reduce the risk of atrial fibrillation and atrial flutter , however, the available data so fare are scanty.

Ankle-Brachial Index: A Simple and Inexpensive Screening Test for Coronary Artery Disease (ABI goes beyond the foot) 踝肱指数:一种简单而廉价的冠状动脉疾病筛查方法(ABI超出足部)

Background: Coronary artery disease (CAD) is the leading cause of death in the world. In this study we assessed ankle-brachial index (ABI) as a screening tool for CAD. Method: Between 2019 and 2020, large cross-sectional population-based study of 4207 new patients referred to cardiovascular Clinic was enrolled. The patients underwent selective coronary angiography via radial artery approach. ABI was calculated for all patients. We compared ABI with the results of coronary angiography to determine the specificity and sensitivity of ABI as a screening tool for CAD. Results: Abnormal ABI was significantly more frequent in patients with proven severe CAD (893, 54.8%) than in patients with proven mild CAD (33, 4.7%) or no CAD (94, 5.3%). The specificity of ABI was 95.3%, and its sensitivity was 54.8%. ABI was associated with risk factors such as smoking, male gender, hypertension, diabetes mellitus and dyslipidemia. Conclusion: ABI can be used as a screening test to rule out CAD with 95.3% specificity. We need to consider risk factors other than ABI to increase screening sensitivity. A multidimensional scoring system should consider risk factors and other noninvasive tests in addition to ABI to develop and ideal screening system for CA. Assessment of Factors Associated with Practice and Knowledge of Essential Newborn Care Among Nurses and Midwives, Western Ethiopia, 2021. 与护士和助产士基本新生儿护理实践和知识相关的因素评估,埃塞俄比亚西部,2021。

Background: Promoting basic newborn care skill and practice is a cost-effective approach to improving newborn health outcomes. Objective: Intended to assess factors associated with Practice of Essential newborn care in western Ethiopia, 2021. Methods: Institutional based cross-sectional study design was conducted. Data was collected by interviewer administered structured questionnaires, entered by Epi-data 3.1version and analyzed by using SPSS version 26. The strength of association between independent and dependent variables was measured by odd ratios with 95% CI at p-value of < 0.05. Conclusion: The Mean value of good score of essential newborn care practice was 41.5% with [95% CI (38.3, 48.5)]. Training, educational qualification, availability of newborn care materials and work experience were significantly associated with knowledge and practice of essential newborn care. Since practice of essential newborn care was low compared to other studies in Ethiopia identified interventional area should be focused by the stake holders.

COVID-19 Pneumonia with Ischemic Heart Event After Neglect Yasser's COVID-19 Discrepancy Phenomenon and Its Regimen; The Seriousness Impact and Deterioration Outcome 忽视后COVID-19肺炎合并缺血性心脏事件Yasser的COVID-19差异现象及其治疗严重性、影响和恶化结果

Rationale: A novel COVID-19 is a multi-systemic serious worldwide pandemic. Undoubtedly, strict management with protocols and guidelines in the COVID-19 pneumonia is remarkable advice but its neglect may be leading to a critical event. Yasser’s COVID-19 Discrepancy phenomenon is a novel descriptive phenomenon that is always seen in all COVID-19 pneumonia. Initial dramatic improvement of the clinical status of COVID-19 pneumonic patient, not a simultaneously after the management, not a coincide with laboratory, radiological, and electrocardiographic workup. Patient Concerns: A 65-year-old married housewife Egyptian female patient was presented to the physician outpatient clinic with COVID-19 pneumonia that deteriorates after early spontaneous stoppage of all COVID-19 medications after the sense of generalized temporary improvement. Diagnosis: COVID-19 pneumonia with angina after neglect Yasser's COVID-19 discrepancy phenomenon and its regimen. Interventions: Electrocardiography, oxygenation, chest CT, and echocardiography. Outcomes: Initial good and dramatic outcome but, the worst outcome with deterioration had happened after early spontaneously stopped all COVID-19 medications. Lessons: The missed time, long-stay hospitalization, patient and governmental economic costs, and psychological stress on both patient and family are hazardous sequels due to neglect to follow Yasser’s COVID-19 Discrepancy phenomenon and its regimen. This is a typical applied example of this phenomenon. Female sex, unstable angina with coronary artery spasm, elevated cardiac biomarkers, a Wavy triple sign (Yasser’s sign) of hypocalcemia and COVID-19 pneumonia represent bad prognostic points and is indicating a high-risk condition.

Endothelial-Mesenchymal Transition (EndoMT) 内皮-间充质转化(EndoMT)

EndoMT is a complex biological process in which endothelial cells lose their specific markers and acquire a mesenchymal phenotype and express mesenchymal cell products. Endo-MT has been reported to occur in fibrosis-related disease. Current study strongly supports the reality of an EndoMT phenomenon in human disease and expands its biological role beyond fibrosis. Recent studies have demonstrated that the occurrence of EndoMT is clearly associated with inflammation and EndoMT is one of the primary drivers of vascular inflammation.
