Carpal kinematics, normal and abnormal, centre on the role of the proximal carpal row, an intercalated segment within the wrist joint. When stability to this segment is impaired by injury or disease, symptoms of weakness, instability or pain may be present, especially with compressive loading of the wrist associated with forearm and hand axial rotation. Treatment of carpal instability is difficult, but can be improved by a systematic approach to the critical factors of acuity, severity, direction, location, aetiology, and pattern. Hand therapists can play an important role in the treatment of carpal instability, and an understanding of the mechanics of carpal instability should help therapists better manage their patients with this difficult problem.

Wrist pain is an all too common and frequently disabling affliction that can defy the attempts of therapists and surgeons to relieve it. An upright posture coupled with the development of the prehensile limb and opposable thumb have conferred great manual dexterity to humans, but mobility has been gained at the expense of stability. We have a tendency to trip and fall over on the outstretched hand causing definable patterns of injury. In addition the wrist is vulnerable to various degenerative disorders most notably osteoarthritis.

Carpal instabilities can be challenging. Undiagnosed wrist pain or dysfunction can have significant impact on function. Therapeutically, in the early stages of mild carpal instability a systematic approach to assessment can provide a basis for a successful outcome. However, there is little research on the success of therapeutic interventions. An approach is offered based on clinical experience and current knowledge of the wrist. Continuing research conducted by therapists is to be encouraged.

Joint dysfunction within the wrist complex can be presented in the hand clinic or outpatient department. Traumatic or insidious in onset, symptoms can be prolonged if intervention is not prompt and appropriate. This article will draw together and integrate various approaches to the use of passive movement as a treatment strategy in the management of joint dysfunction at the wrist. In an attempt to most effectively restore full functional and pain free mobility of the wrist complex clues from the subjective and objective examination will be highlighted to assist in the initial selection and later progression of passive movements as a mobilisation technique. Rather than a literature review of the efficacy of passive movements, this is a practical guide for application to clinical practice.
