Diagnosis and Treatment of Cervical Intraepithelial Neoplasia 宫颈上皮内瘤变的诊断与治疗

Objective To investigate the diagnosis and treatment of CIN.Methods One hundred and fifty inpatients with CIN treated in our hospital from January,1984 to December,1998 were retrospectively analyzed.Results The positive rate of cytology was 76.6%,which was 78.8% when combined with endocervical cytology( P 0.05).The positive rate of cytology combining colposcopically biopsy(95.3%) to diagnose CIN was significantly higher than that of cytology.The global agreement between colposcopically directed biopsy and final diagnosis was observed in 91.6%.141 patients underwent surgery with no positive margin,9 carcinoma in situ received intracavity radiotherapy alone.None of the patients recurred or died of the disease.Conclusion Cytology combining colposcopically directed biopsy is an effective method to diagnose CINs,ECC should be routinely performed in postmenopausal women.Intracavity radiotherapy alone may be as an alternative treatment for medically inoperable patient with carcinoma in situ of the cervix.
目的探讨CIN的诊断和治疗方法。方法对我院1984年1月~ 1998年12月收治的150例CIN住院患者进行回顾性分析。结果宫颈细胞学检查阳性率为76.6%,宫颈细胞学检查阳性率为78.8% (P < 0.05)。细胞学结合阴道镜活检诊断CIN的阳性率(95.3%)明显高于细胞学。91.6%的患者在阴道镜指导下活检与最终诊断之间达成了一致。141例手术无阳性切缘,9例原位癌单独行腔内放疗。所有患者均无复发或死亡。结论细胞学联合阴道镜下活检是诊断CINs的有效方法,绝经后妇女应常规行ECC检查。单纯腔内放疗可作为医学上不能手术的宫颈原位癌患者的替代治疗方法。
