A 37-year-old female presented with persistent dyspnea and syncope. Computed-tomography scan detected saddle pulmonary embolism, and echocardiogram revealed extensive thrombus crossing the interatrial septum and prolapsing across the mitral valve into the left ventricle. Consequently, the patient was treated with surgical extraction of the thrombus and pulmonary embolectomy. She was discharged after un-eventful hospital stay save a dark grey spot in her central vision, which was diagnosed as homonymous paracentral scotoma caused by pre-operative cerebral infarction.

With magnesium being the second most common intracellular cation in the human body and the presence of hypomagnesemia in 10% of hospital admissions, magnesium deficiency has become a clinically significant nutritional deficiency in context of heart health. Magnesium has long been used as a treatment modality for cardiac arrhythmias as serum magnesium concentration of 1.44 mEq/L to 1.8 mEq/L has been found to have a statistically significant inverse association with cardiovascular disease events. Patients with the highest intake of magnesium were found to have incidents of sudden cardiac death reduced by 77% while hypomagnesemia is found to be associated with increased risk of congestive heart failure and cardiac arrhythmias. It has also been demonstrated that high dietary intake of serum magnesium is associated with decrease in systolic and diastolic blood pressure secondary to reduction in vascular tone and an increase in vasodilatory substances, like Prostaglandin E. Association of cardiovascular diseases with hypomagnesemia and the need for the fulfilment of RDA goals, authenticates the use of oral magnesium supplements, with established safety profile and lack of potential for toxicity.

Malignant neoplasms that metastasize to the head and neck remain a rare occurrence. Metastatic lesions to the tongue are even more rarely encountered. The majority of the few reported cases have primary sites in the lung, breast, and kidney. Adenocarcinoma of the esophagus with isolated metastasis to the tongue is a unique occurrence with only a handful of cases with other extensive metastases ever reported. Herein we present the case of a patient with adenocarcinoma of the distal third esophagus who underwent a neoadjuvant therapy followed by margin negative esophageal resection, including negative lymph nodes, who subsequently presented with a solitary distant metastasis to the tongue.

Breast cancer is the main type of cancer that affects women in the world. Treatment with chemotherapeutic agents for this type of cancer depends to a large extent on the phenotypic characteristics that appear in the tumor, such as some receptors. The treatment of breast cancer is complex since not all patients respond adequately to it and this is partly due to mechanisms of resistance to treatment. The resistance either acquired or intrinsic to chemotherapeutics against breast cancer is related to multiple genes and proteins that are actively involved in the development of resistance mechanisms, which may be inhibiting the binding of the drug to the target receptor, inhibiting the gene expression of the receptors, activating signaling pathways, inducing the action of transporters that expel the drug from the cell, etc. As a result of this review, the mechanisms of chemotherapeutic resistance in breast cancer derived from genes and proteins related to say disease are described, including genes related to different types of breast cancer, as well as with different therapeutic strategies. These genes and their products of expression include a wide range of interactions and activations of signalling pathways that trigger a poor response to treatment, which translates into a worse prognosis, higher risk of prevalence and death, favouring breast cancer to be the main cause of cancer death in women. Objective: Conduct an updated review of genes and proteins, that have been associated with conferring resistance to the different chemotherapeutic agents used for the treatment of breast cancer, including the mechanism of resistance and the type of cancer in which it is generated. Method: The search for indexed articles in the PubMed (https://www.ncbi.nlm.nih.gov/pubmed/) and ScienceDirect (http://www.sciencedirect.com) databases was performed, using the words “Breast cancer resistance” as a search criterion, articles published in the period 2015-2017 were selected.
