The possible use of dalbavancin as a catheter lock solution was previously demonstrated by our study group. However, it was needed to assess whether heparin could affect dalbavancin bioactivity during freezing storage. We tested the bioactivity of a dalbavancin+heparin (DH) vs. dalbavancin () against Staphylococcal biofilms comparing DH median value of cfu counts and metabolic activity with that obtained for before and during storage under freezing up to 6 months. Despite there was a slight decrease in the median percentage reduction of metabolic activity at month 3 in between DH and (97.6 vs. 100, =0.037), considering the clinical criteria, no significant reduction in any of the variables tested was observed at the end of the experiment between and DH solutions. The addition of heparin to a dalbavancin lock solution did not affect its bioactivity against staphylococcal biofilms irrespective of its preservation time under freezing.

En España, la mitad de los nuevos diagnósticos del virus de la inmunodeficiencia humana (VIH) son tardíos y una proporción significativa de las personas que viven con el VIH (PVVIH) aún no se ha diagnosticado. Nuestro objetivo fue evaluar la efectividad de una estrategia de cribado oportunista automatizado del VIH en el entorno hospitalario. Entre abril de 2022 y septiembre de 2023, se realizó la prueba del VIH a todos los pacientes en que se solicitó un perfil analítico de ingreso hospitalario, un perfil prequirúrgico y en varios perfiles serológicos prediseñados (fiebre de origen desconocido, neumonía, mononucleosis, hepatitis, infección de transmisión sexual, exantema, endocarditis y miopericarditis). Se estableció un circuito de derivación de pacientes diagnosticados al especialista. Se realizaron 6.407 pruebas de VIH incluidas en los perfiles y se diagnosticaron 18 (0,3%) nuevos casos (26,4% de todos los diagnósticos del área sanitaria). Cinco pacientes se diagnosticaron por perfil de ingreso hospitalario o prequirúrgico y 13 por un perfil serológico solicitado por entidades indicadoras (fiebre de origen desconocido, infección de transmisión sexual, mononucleosis) o posiblemente asociadas (neumonía) a infección oculta. En cinco (27,8%) pacientes se documentó una infección reciente y en nueve (50,0%) un diagnóstico tardío, de los que en cinco (55,5%) se había perdido la oportunidad de ser diagnosticados previamente. Este cribado oportunista fue rentable ya que la tasa de positivos del 0,3% es coste-efectiva y permitió realizar la cuarta parte de los nuevos diagnósticos, por lo que parece una buena estrategia que contribuye a disminuir la infección oculta y el diagnóstico tardío.

Introduction
The COVID-19 pandemic caused by the SARS-CoV-2 virus greatly affected healthcare workers and healthcare systems. It also challenged schools and universities worldwide negatively affecting in-person education. We conducted this study is to assess the evolution of SARs-CoV-2 virulence over the course of the pandemic.
Methods
A combined cohort of University students in Spain and HCWs from the two hospitals in Spain, and one healthcare system in the Greater Boston area was followed prospectively from March 8th, 2020, to January 31st, 2022 for diagnosis with COVID-19 by PCR testing and related sequelae. Follow-up time was divided into four periods according to distinct waves of infection during the pandemic. Severity of COVID-19 was measured by case-hospitalization rate. Descriptive statistics and multivariable-adjusted statistics using the Poisson mixed-effects regression model were applied. As a sensitivity analysis, information on SARS-CoV-2 RNA in wastewater and COVID-19 deaths through May 30, 2023 from the Boston area was collected.
Results
For the last two periods of the study (January 1st to December 15th, 2021 and December 16th, 2021 to January 31st, 2022) and relative to the first period (March 8th to May 31st, 2020), the incidence rate ratios (IRRs) of hospitalization were 0.08 (95% CI, 0.03–0.17) and 0.03 (95% CI, 0.01–0.15), respectively. In addition, a relative risk 0.012 CI95% (0.012–0.012) was observed when comparing COVID-19 mortality versus SARS-CoV-2 RNA copies/mL in Boston-area wastewater over the period (16th December 2021 to 30th May 2023) and relative to the first period.
Conclusions
The severity of COVID-19 and immunity of our populations evolved over time, resulting in a decrease in case severity. We found the case-hospitalization rate decreased more than 90% in our cohort despite an increase in incidence.
