Email is a major means of communication in healthcare and it facilitates the fast delivery of messages and information. But email's ubiquity has brought challenges. It has changed the way we get things done, and working days can be dictated by the receipt and reply of multiple email messages, which drown out other priorities. This article examines email's advantages and disadvantages and, with a focus on healthcare professionals, examines what individuals and organisations can do to ensure email works for us, rather than – as can seem the case – the other way around.

Essays on Partial Derangement of the Mind in Supposed Connexion with Religion by John Cheyne 关于假定与宗教有关的部分精神错乱的论文》,约翰-切恩著
Pub Date : 2024-01-25 DOI: 10.1192/bja.2023.73
Christopher C. H. Cook
Essays on Partial Derangement of the Mind in Supposed Connection with Religion, written amidst the illness of its author almost two centuries ago, was the little-known work of a distinguished physician. Seeking to rebut the argument that religion is causative of ‘derangement of the mind’ it takes a surprisingly biological view of such conditions while, at the same time, affirming the importance of faith, hope and love in human well-being. Despite its limitations, it makes observations that remain relevant to debates about religion and psychiatry today.

Cognitive–behavioural therapy as a comprehensive treatment for personality disorders 认知行为疗法作为人格障碍的综合疗法
Pub Date : 2023-11-22 DOI: 10.1192/bja.2023.59
D. Carreiras, Paula Castilho, Daniel Rijo, M. Salvador, C. Carona
Personality disorders are a group of psychological disorders characterised by a developmental nature, long-lasting impairment and emotional suffering. Personality disorders have an estimated prevalence rate of approximately 8% in community settings, but in in-patient settings the rate might be as high as 76%. Cognitive–behavioural therapies (CBTs) include psychotherapies that emphasise the identification and modification of maladaptive thought patterns and behaviours that contribute to the maintenance of psychological disorders. CBTs have demonstrated their effectiveness in treating various types of personality disorder. This article focuses on the nature of personality disorders and their categorial and dimensional assessment and neurobiology. We present three influential CBT models used in personality disorders: schema therapy, cognitive interpersonal therapy and dialectical behaviour therapy. For each one, we outline the rationale, intervention strategies and therapeutic techniques, with practical examples and summary tables to illustrate their application.

Pub Date : 2023-11-17 DOI: 10.1192/bja.2023.60

Cold-water immersion (e.g. adapted cold showers, partial or whole-body immersion, cold swimming) are nowadays increasingly being used as an adjunctive procedure to enhance the effects of primary treatment of various clinical conditions, including depressive and anxiety disorders. This brief article reviews the evidence regarding the beneficial effects of cold-water immersion on clinical depression and anxiety and outlines potential therapeutic mechanisms underlying the intervention. Promising avenues for future research and best practice recommendations are also discussed to improve the clinical effectiveness of cold-water immersion.
