Atypical hemolytic-uremic syndrome in pregnancy: a case report 妊娠期非典型溶血尿毒综合征:病例报告

Introduction: The diagnosis of Atypical hemolytic-uremic syndrome (aHUS) is largely imprecise. Improving outcomes requires accurate diagnosis and timely management. Acute kidney damage, thrombocytopenia, and microangiopathic hemolytic anemia are all signs of aHUS. The condition is brought on by pregnancy, and in genetically susceptible women, it progresses to a terrible hemolytic illness marked by widespread endothelium damage and platelet consumption. The sickness is a potentially fatal ailment that demands quick identification and treatment.Case Presentation: Our facility provided treatment for severe anemia, thrombocytopenia, and acute renal damage in a 24-year-old G1P1A0 postpartum lady with Caesarean sectio and a HELLP syndrome suspicion. An aHUS diagnosis was later verified. The condition of the patient failed to improve in the first 24 hours after birth. Inside this presence of TMA, the patient began on daily TPE and ran in parallel prednisone medication (1 mg/kg/day). After six TPE cycles, the laboratory values began to rise.Conclusion: AHUS can be challenging to diagnose early since it frequently mimics other illnesses. To enhance results, proper diagnosis and prompt management are essential. The management strategy includes a multidisciplinary team, early plasmapheresis, and complement inhibition. To lessen the effects of aHUS, TPE should be carried out as soon as feasible on a daily basis.

The relationship between the ratio of midwives and antenatal care coverage in Central Kalimantan in 2020-2022 2020-2022 年中加里曼丹助产士比例与产前护理覆盖率之间的关系

Introduction : Antenatal care (ANC) is a healthcare service for pregnant women. The quality of health services is heavily influenced by the performance of the health workers, including midwives. In Central Kalimantan, in 2021, the first ANC coverage (K1) achieved 98.15%, while the fourth coverage (K4) was at 88.14%. Four districts/cities in Central Kalimantan had public health centers with midwife ratios below the minimum requirement (130 midwives per 100,000 population). The purpose of the study was to determine the relationship between the ratio of midwives and the ANC coverage in Central Kalimantan in 2020–2022.Methods : The data was analyzed using the cross-sectional study design. The dependent variable was the ANC coverage in Central Kalimantan in 2020–2022, while the independent variable was the ratio of midwives in Central Kalimantan in 2020–2022.Results : In Central Kalimantan, 10 (71.4%) districts had achieved the target midwife ratio, while 4 (28.6%) had not. There had been a yearly decrease from K1 to K4. Despite positive growth in midwife ratios, there was no significant correlation between midwife ratios and ANC coverage for both K1 and K4.Conclusion : The ratio of midwives had not significantly contributed to the coverage of ANC K1 and K4 in Central Kalimantan. Various factors that could impact the ANC coverage, aside from human resources, must be taken into account. It is crucial for the government to expand its involvement.

Spontaneous hydropneumothorax with unknown origin and new onset hyperosmolar hyperglycemic state: case report in limited health facility 原因不明的自发性气胸和新发的高渗性高血糖状态:有限医疗机构的病例报告

Introduction: Hydropneumothorax and Hyperosmolar Hyperglycaemic State (HHS) are uncommon disorders with serious mortality. Both conditions need appropriate intervention and monitoring in comprehensive health facilities. Reports on the occurrence of spontaneous hydropneumothorax accompanied by HHS are still limited, due to the rarity of these two disorders.Case Presentation: A 42-year-old woman with abrupt shortness of breath started 1 day ago. No coughing or chest pain. Losing weight in last six months. There was no history of any disease and medication. Physical findings were drowsy consciousness with tachycardia and weak pulse, asymmetrical chest expansion with retraction, vesicular breath sounds diminished in the left lung following hyper resonant in percussion. The extremities were cold and dry. Skin turgor was reduced. Plain CXR revealed air and fluid in left pleural cavity. Laboratory test revealed very high blood glucose with hyperosmolarity.Discussion: Hydropneumothorax is the presence of air in the pleural space accompanied by ipsilateral effusion. The manifestations include shortness of breath, increased work of breathing, and sharp chest pain, or asymptomatic. It is unclear whether this patient's hyperglycaemia is related to the occurrence of spontaneous hydropneumothorax. HHS can be diagnosed by effective serum osmolality with glucose levels plus appropriate medical presentation. The management is based on the correction of dehydration, hyperglycaemia, hyperosmolarity, and precipitating conditions.Conclusion: Both hydropneumothorax and HHS have high mortality rates, particularly when the two conditions coexist. This case met some challenging issues that might increase the mortality.

Introduction: Nutrition is an organism's process of using food that is normally consumed through the process of digestion, absorption, transportation, storage, metabolism and excretion of substances that are not used to maintain life, growth and normal function of organs and produce energy.Main Text: Mother's milk is the right food source for babies, especially in the first month, which contains many important nutrients needed by babies to provide energy. Balanced nutrition for breastfeeding mothers must meet the needs for themselves and for the growth and development of the baby. Fulfillment of nutrition for breastfeeding mothers is the principle of fulfilling balanced nutrition. Balanced nutrition during breastfeeding is a very important requirement for breastfeeding mothers.Conclusion: Nutrition in nursing mothers is closely related to the production of breast milk which is needed for the growth and development of the baby.

The relationship between daytime napping and anxiety level of medical faculty students of universitas Muhammadiyah Purwokerto 穆罕默迪亚大学医学系学生白天小睡与焦虑水平之间的关系

Introduction: Anxiety and sleep deprivation are common concerns among medical students, affecting their essential cognitive and psychomotor skills. One effective solution is incorporating daytime napping, which improves performance, productivity, and mental well-being. Objectives: Analyzing the relationship between the frequency and duration of naps and the level of anxiety among students of the Faculty of Medicine, Universitas Muhammadiyah Purwokerto. Methods Materials: A cross-sectional study was conducted on medical students at the Faculty of Medicine, Universitas Muhammadiyah Purwokerto. The quality of daytime napping was assessed based on a questionnaire about the frequency and duration of daytime naps, and the anxiety levels of the respondents were evaluated using the Hamilton Anxiety Rating Scale (HARS). Results: Among 185 respondents, the majority had a daytime napping habit (93%), with a frequency of 1-3 times per week (56.2%), lasting for 31-60 minutes per day (44.9%). Fisher's analysis results indicate that respondents with a daytime napping habit with a frequency of 4-6 times (p = 0.018) and a duration of 1-30 minutes per week (p = 0.022) tend to have lower anxiety levels. Conclusion: Based on the analysis and discussion conducted, it can be concluded that there is a very strong relationship between daytime napping habits and anxiety levels in the Faculty of Medicine, Muhammadiyah Purwokerto University students.

Introduction: Leprosy is a chronic infectious disease caused by the bacterium Mycobacterium leprae. Leprosy causes not only medical problems but also social problems such as stigma. Depression in leprosy patients is thought to be related to stigma from society and from leprosy sufferers, who find it difficult to accept their disability.Methods: We conducted this systematic review by gathering articles from the PubMed, ProQuest, and Google Scholar databases Results: At the stage of article collection, 1558 articles were obtained from the databases. After screening, the author finally obtained seven articles for review.Conclusions: Stigma (enacted, perceived and internalized stigma) is associated with the incidence of depression in leprosy patients.
