The Single Cusp of Life: A Case Report 生命的一个转折点:一个案例报告

Background: Anomalous coronary artery (CAA) is a rare condition occur-ring in less than 1% of individuals. The most common form consists of the left circumflex artery (LCx) arising from the right coronary sinus. These vascular anomalies have been associated with an increased risk of sudden cardiac death. We present a rare case of an anomalous coronary artery in a patient with all three coronary arteries arising from a shared ostium. Case presentation: A 67-year-old transgender Caucasian male with medical history of dys-lipidemia and hypertension presented for ischemic workup due to occasional chest pain prior to undergoing male to female gender reassignment surgery. A Regadenosan stress test with SPECT myocardial perfusion imaging revealed moderate sized, moderate intensity perfusion defect reversible in inferolateral wall and fixed in inferior wall. Coronary angiography revealed anomalous origin of the coronary arteries, with all three major coronary arteries arising from a shared, single, ostium originating from the right coronary cusp. Coronary computed tomographic angiography (CCTA) showed a “malignant course” of LAD running between the pulmonary artery and aorta. A conservative management was pursued in view of mild symptoms with close follow ups. Conclusion: It is important to evaluate all coronary artery anomalies (CAA) for a “malignant” course due to its associated risk for various cardiac events including sudden cardiac death. Surgical management is indicated in high-risk patients with malignant courses. However, asymptomatic patients and those with mild symptoms with or without malignant course can be followed closely.

Right Coronary Artery Fistulae Draining into Right Atrium: A Case Report 右冠状动脉瘘管引流至右心房1例

We report the case of a 16-year-old young girl seen for precordial pain and stage II NYHA dyspnea. Clinical examination found a maximal systolodias-tolic murmur in the left subclavicular. Lateral Q waves were noted on the electrocardiogram and echocardiography revealed a right coronary fistula draining in right atrium. CT scan confirmed the diagnosis. Patient was addressed to a specialized center for percutaneous closure of the fistula. In case of signs indicating coronary artery disease, the search for coronary anomalies should be systematic in children by using EKG and transthoracic echocardiography. In case of doubt, a second imaging such as the multislice Ct scan is performed. Coronary angiography has a dual diagnostic and therapeutic interest. The indications for treatment depend on the symptoms and the results of ischemia tests.

SARS-COV-2 Induce Pulmonary Injury from Basic to Clinical Research SARS-COV-2诱导肺损伤从基础研究到临床研究

The SARS-CoV-2 has infected over 194,909,000 cases and over 4,170,000 deaths in the world before the end of July 2021. The virus attacks human alveoli and induces severe lung injury (COVID-19 disease) and spreads rapidly. The mechanisms of COVID-19 disease are unclear. To better understand this disease, This review analyzes the SARS-CoV-2 biological characteristics, in-sights the effect of alveolar epithelium and its adjacent microvascular endothelium, investigates human host cells immune response and immunothrombosis. Explains clinical manifestations of COVID-19 associated lung injury. It may be helpful for development management strategies for COVID-19 associated pulmonary damage.
