Hybrid SPECT/CT for Tracking of Mesenchymal Stem Cells after Intravenous and Intra-Thecal Administration in Acute Cervical Traumatic SpinalCord Injury
Pub Date : 2022-04-19 DOI: 10.36266/ijn/143
Introduction: Mesenchymal stem cells (MSCs) are a promising strategy for spinal cord repairafter traumatic spinal cord injury (tSCI), but their ability to migrate, engraft, and survive at injury sites has yet to be investigated in humans.Case Presentation: In this case report, using hybrid SPECT/CT, we found that radiolabeled autologous bone marrow-derived MSCs after intravenous administration distributed in the reticuloendothelial system and lung parenchyma, but accumulated unremarkably at the site of tSCI, whereas after intrathecal administration they spread rostraly in the cerebrospinal fluid and accumulated at the site of tSCI as early as 1 hour after administration, with increasing accumulation at 4 hours. The migration and homing potential of MSCs after intrathecal administration was maintained even one and a half years after tSCI. We observed no serious side effects during and after the procedure, however, neurologic recovery was minimal and could not be attributed solely to MSCs administration.Discussion: This case suggests that hybrid SPECT/CT imaging could be successfully used to track radiolabeled MSCs within the first day after intravenous and intrathecal administration of MSCs in a patient with tSCI without serious side effects.

Introduction: Recent evidence has demonstrated a close relationship between the cerebral venous and lymphatic systems. Venous congestion has been implicated in a host of neurologic disorders, with relevance for vascular etiologies.

Objective: The authors aim to review the literature as it pertains to brain arteriovenous malformations' (BAVMs) venous hemodynamics and glymphatic pathways, as well as the implications of BAVM treatment.

Results: BAVMs offer a unique challenge, with sudden alteration in flow dynamics leading to increased hemorrhage risk and difficult challenges post-treatment.

Conclusion: Recent progress in the understanding of CNS fluid dynamics and glymphatic pathways have revealed important implications for BAVM pathology and treatment. As imaging techniques and treatment modalities advance, there is a need to further investigate this relationship as it relates to therapeutic options and post-treatment sequalae.

Morphology of the Dural Venous Complex of Skull Base in Human Ontogenesis

The development of the dural venous complex of the skull base formed by the cavernous, intercavernous, and petrous dural sinuses and their connections with the intra- and extracranial veins and venous plexuses, was investigated on 112 premature stillborn human fetuses from 16 to 36 weeks of gestation by methods of vascular corrosion casting. It was established that the main intracranial dural canals approach similar to the mature arrangement at the very beginning of the early fetal period. In fetuses 16 weeks of gestation, the parasellar dural venous complex appeared as a plexiform venous ring draining the venous plexus of the orbits into the petrous sinuses. The average diameter of dural canals progressively enlarged and reached its maximum value 2.2 ± 0.53 mm approaching the 24 th week of gestation. This developmental stage is characterized by the intensive formation of the emissary veins connecting the cavernous sinus with the extracranial venous plexuses. Due to the particular fusion of the intraluminal canals, the average diameter of the lumen gradually declined to reach 1.9 ± 0.54 mm in 36-week-old fetuses. By the end of the fetal development, 21.3% of fetuses featured a considerable reduction of the primary venous system with the formation of the one-canal shaped dural venous sinuses, obliteration of several tributaries, and decreased density of the extracranial venous plexuses. In the other third of fetuses, the enhanced venous basal complex with the multi-canal shape of dural sinuses, and abundance of tributaries and anastomoses persisted thought the whole antenatal period. Thus, the genetically determined pace of reduction of the primary venous plexuses determines the final shape of the dural venous system and its compensatory abilities.
