Purpose: Osteoporosis is a major risk factor for the development of fragility fractures of the pelvis (FFP). There is a lack of information about the influence of anatomical conditions such as Pelvic Incidence and Pelvic Ratio (DT/DS ratio) on this kind of fractures. Methods: This is a monocentric retrospective analysis. X-ray images of the lumbar spine and pelvis and 3D-MPR CT reconstructions of the pelvis were analysed to determine Pelvic Incidence (PI) and Pelvic Ratio (PR) in 141 fragility fractures of the pelvis. Statistical analyses were performed to examine the correlation between these spinopelvic parameters and fragility fractures of the pelvis. Results: A total of 141 fragility fractures of the pelvis (14 men = 9.93%, 127 women = 90.07%) were analysed. According to the FFP-classification we recognized FFP type 1 fractures in 19.15%, FFP type 2 in 41.13%, FFP type 3 in 8.51% and FFP type 4 fractures in 32.21%. The mean PI was 58.83º. There was no statistical correlation between PI and fracture types (p=0.81). The mean PR was 1.099. 57 patients (40.43%) demonstrated a DT/DS ratio ≤ 1.06, corresponding to a circle-type morphology. 24 patients (17.02%) demonstrated a DT/DS ratio ≥ 1.18, corresponding to an ellipse-type pelvis. A circle-type pelvis is significantly more often associated with fragility fractures of the pelvis than an ellipse-type morphology (p<0.001). Conclusion: The results of our work demonstrate a strong statistical correlation between the circle-type morphology of the pelvis (PR ≤ 1.06) and fragility fractures of the pelvis. There is no statistical correlation between fragility fractures of the pelvis and Pelvic Incidence.

The increase in consultations for changes and/or cognitive complaints in the elderly, together with the current interest in epidemiological research in this context creates the need for screening tools for cognitive assessment to enable the detection of early deficits. Evidence shows its predictive value in the development of dementia disease. This study aims at displaying the results of a Cognitive Skills Questionnaire (CSQ) in a patient population with mild cognitive impairment (MCI) and Alzheimer’s disease (AD), both compared with a control group (CG) with no cognitive disorder and verifying its sensitivity and specificity in order to identify risk patients with cognitive disorder. Participants and Methods: A total of 208 participants were evaluated, out of which 60 had MCI, 46 had AD and a remaining group of 102 subjects who had no cognitive disorder. All participants were administrated the CSQ and a battery of neuropsychological proofs. We analysed the statistical data using ANOVA, Student’s t-test, Tuckey test, ROC curve and principal components analysis. A multiple regression analysis was carried out so as to single out those questions which better differentiated the studied groups. Results: The CSQ showed significant differences between the CG and both groups of patients (AD p> 0.01 and MCI p> 0.05). It was established a cut-off point of 17.5 in the CSQ total score with a sensitivity of 93% and a specificity of 91.3%. Conclusion: The CSQ could eventually allow us to identify patients with cognitive disorders and those others with a cognitive complaint greater than expected. Thus, this questionnaire could be a useful testing and counselling tool in health primary attention.

Purpose: The purpose of the study is to analyze the structure of mental disorders and social adaptation in retired ex-combatants of the security forces. Methods: 111 retired men, ex-combatants who sought medical care, were examined. There are two groups: I: 56 people with identified mental disorders; II: 55 with pre-nosological mental health disorders. Results: It is shown that the state of mental health of retired ex-combatants is characterized by severe polymorphic mental disorders, especially those expressed in group I (organic, behavioural disorders, substance use, schizophrenia, schizotypic and delusional disorders, affective, neurotic, stress-related, and somatoform disorders, personality and behaviour disorders in adulthood), as well as social maladaptation, associated with the transformation of values, lifestyle and communication, the emergence of difficulties in social and everyday life, the breakdown of social ties and the displacement of pensioners from public life. Conclusion: The article substantiates the need for a comprehensive approach to the rehabilitation of retired ex-combatants with the interaction of all government structures, public associations, medical, psychological and social institutions directly related to the development and implementation of individual rehabilitation routes.

Introduction: Tuberculosis (TB) is an infectious disease from which people suffer even in the 21st Century. According to official data for Croatia, the number of cases in the last decade has been decreasing. Aim: Due to the triple reduction in incidence in the last ten years in Croatia, the purpose of this study is to investigate whether patient characteristics have been changed, along with their comorbidities. Material and Methods: This was a cross-sectional study where we treated 230 patients (62.1% male and 37.8% female) age 40-60 years with tuberculosis in 2003 and in 2013, we treated 70 patients (54.9% of men and 45% women). Tuberculosis was diagnosed by direct microscopy of sputum, catheter aspirate, tumor tissue, lymph node puncture, swab wound in 2003 and by direct microscopy of sputum samples, catheter aspirates and bronchial washing samples in 50% of cases in 2013. Results: In 2003, 95.2% of patients had pulmonary tuberculosis. Comorbidities were found in 63.8%. 51.3% were smokers, 36.5% were alcoholics, diabetes and chronic obstructive pulmonary disease (COPD) was found in 12.2% and tumors in 6.5%. After 10 years, 91.4% of patients had pulmonary tuberculosis. 90% of them had comorbidities - 14.2% of patients had diabetes mellitus, 10% were treated for malignancies, COPD was present in 8.5%. 31.4% were smokers, while alcoholism was found in 11.4%. Conclusion: After 10 years tuberculosis has changed its face in South-East Europe. TB occurs more often in the elderly population with comorbidities in 90% of cases (mostly diabetes mellitus, malignancies and COPD). Men are no longer a risk group because women are affected almost equally. Clinical awareness of the possibility of TB should be considered in patients with comorbidities, and active screening and prevention should be undertaken

This case report highlights problems encountered by psychiatrists when treating a 68-year-old female patient who presented with a first episode of psychosis. She suffered from constipation, an anticholinergic side effect of quetiapine and both anticholinergic and extrapyramidal side effects of olanzapine. Finally, she was able to tolerate a combination of two pharmacologically different antipsychotics namely olanzapine and aripiprazole combined with a course of ECT followed by maintenance ECT. The authors would like to highlight maintenance ECT as part of the solution to patients who find it difficult to tolerate antipsychotics. Especially when only low doses of antipsychotics can be tolerated by the patient.
