It is possible to manipulate myocardial metabolism to improve recovery after ischaemic insults. We studied 2 metabolic manipulations: addition of amino acids during cardioplegia, and treatment with orotic acid before cardioplegia. Aspartate improved recovery after cardioplegic arrest, and glutamate exerted a minimal protective effect. We studied the recently infarcted heart to determine whether orotic acid therapy could improve recovery after cardioplegia. The infarct placed the non-infarcted myocardium under stress and increased its sensitivity to hypothermic cardioplegia. This sensitivity was reduced by treatment with orotic acid. Metabolic supplementation may be useful during cardiac surgery, after myocardial infarction, and in the early period after cardiac surgery.

From February 1981 to December 1991, 152 patients had surgery for supraventricular arrhythmias; 118 had division of an accessory atrioventricular connection (AAVC) for Wolff Parkinson-White syndrome. At the first operation, 128 of 133 pathways were divided, and 112 patients were asymptomatic. Five patients had a second, successful procedure. No patients died. A procedure for partial isolation of the atrioventricular (AV) node was performed in 15 patients; 14 were cured. There were 2 late deaths. Four patients had division of an AAVC and AV node re-entry; all were cured. In our early experience, 10 patients had ablation of the atrioventricular node, and all the procedures were successful.

We have performed in Perth 153 coronary artery bypass operations using the right gastroepiploic artery (RGE), almost always in association with internal mammary artery grafts. Use of the RGE increases operative complexity and morbidity. We limit use of the RGE to situations where no other suitable conduit is available.

Resection remains an important part of treating oesophageal carcinoma. The type of operation may not be as important as selecting the correct patient. Long-term survival will improve when systemic treatment is combined with surgery.
