The 21st century has brought forth many significant advances in science and technology within the field of dentistry, in addition to a wide range of novel treatment modalities that are available for dentists to use. Photodynamic therapy (PDT) is an emerging, non-invasive treatment method, involving photosensitizers, light of a specific wavelength and the generation of singlet oxygen and reactive oxygen species (ROS) to eliminate unwanted eukaryotic cells (e.g., malignancies in the oral cavity) or pathogenic microorganisms. In the present chapter, we aimed to summarize the history and the understanding of PDT, general concepts and the wide-ranging uses of PDT in many fields of dentistry (oral and maxillofacial surgery, oral medicine, endodontics, preventive dentistry, periodontology and implantology), highlighting advantages, drawbacks and future perspectives regarding this method.

Introduction: Pyogenic granuloma is a benign lesion of unknown etiology that occurs in the skin and mucous membrane. It has been reported as a common lesion in oral cavity and less commonly in nasal cavity. Although its etiology is unclear, pyogenic granuloma is commonly associated with pregnancy, oral contraceptives and trauma. Objective: To diagnose and treat appropriately the epistaxis during pregnancy considering maternal and fetal safety. Case: A 22 year old female patient with 7 months of amenorrhoea came to ENT opd with complaints of epistaxis and swelling in left nasal cavity ultimately diagnosed as pyogenic granuloma. The patient was treated with endoscopic excision of nasal mass under general anesthesia. The management of pregnant female with such lesion was challenging, so we had to operate her. Even radiation exposure was a risk while radioimaging before surgery. Conclusion: Pyogenic granuloma is rapidly growing lesion that should be considered in the differential diagnosis of gravid patient with oral and nasal mass. Early excision of mass helped in avoidance of further complications such as fetal distress, low birth weight and reduces maternal mortality and morbidity.

It is well known to the general population that transfusion of blood and blood products can save lives, but also that this transfusion can cause serious risks of transmitting infections. Traditionally, Jehovah's Witnesses are deeply religious people who refuse blood transfusion. The 2010 census in Brazil indicates that this year there were 1,393,208 Jehovah's Witnesses in Brazil. Most anesthesiologists consulted do not respect the wishes of Jehovah's Witness patients. Although the Brazilian Constitution guarantees religious freedom and the autonomy of life, the jurisprudence considers that people are not authorized to have their own lives for religious reasons. In the disciplinary and judicial administrative fields, there is a current scenario that makes room for the responsibility of the professional and the hospital. The two cases, a 17-year-old man and a 77-year-old woman, withstand bloodless treatment with Hb of around 4 g/dL. Multiple strategies to prevent and optimize the oxygen supply will decrease the incidence of anemia and decrease the morbidity and mortality of Jehovah's Witness patients. The medical team must respect the patients' autonomy and adhere to the principles of bioethics such as beneficence, non-maleficence and sometimes justice are also fundamental. The main objective of the chapter was to evaluate the Jurisprudence of the subject, the degree of knowledge of anesthesiologists in relation to the treatment of Jehovah's Witnesses, through a questionnaire. As a secondary objective, it was to report the experience with two patients, one young and one elderly, in surgical anesthetic management.

Hypertensive disorders of pregnancy constitute one of the leading causes of maternal and perinatal mortality worldwide. Chronic subdural hematoma is extremely uncommon in Pregnancy. Intracranial haemorrhage (ICH) is deadly complication, if it occurs in pregnancy both mother and fetus gets compromised. Intracranial bleeding is not uncommon in pregnancy, early diagnosis can save mother from mortality. Here we present a rare case of Subdural Hematoma in a patient with eclampsia in pregnancy, in which both the mother and baby were discharged with a multidisciplinary approach and early detection of the condition.

DNA sequencing is the method of identifying the precise order of DNA nucleotides within a molecule. The information of DNA sequencing is of prime requisite for basic biological research as well as in various clinical specialties. They can be used to determine the individual genetic sequence, larger genetic regions, chromosomes as well as to sequence RNA and proteins. Since the first DNA sequencing in 1970s, there has been tremendous advancements in the technologies aimed to determine the entire human genome. The need for rapid and accurate sequencing of human genome has resulted in the introduction of next generation sequencing technology. Next generation sequencing refers to the second-generation DNA sequencing technologies where millions of DNA can be sequenced simultaneously. Some of the next gen sequencing methods employed are Roche/454 life science, Illumina/ Solexa, SOLiD system and HeliScope. Application of next generation sequencing in decoding the genomic database of various oral diseases may possess therapeutic and prognostic value. The objective of this review is to provide an advanced insight into the applications of next generation sequencing in oral disease diagnostics.
采用的新一代测序方法有Roche/454 life science、Illumina/ Solexa、SOLiD system和HeliScope。下一代测序技术在各种口腔疾病基因组数据库解码中的应用可能具有治疗和预后价值。这篇综述的目的是为下一代测序在口腔疾病诊断中的应用提供一个先进的见解。

Pancreatic pseudocysts are frequently encountered in various clinical settings. Surgical treatment is indicated in cases with severe and persistant symptoms, complications or a high suspicion of malignancy. There are many therapeutical options available with no prospective and randomised comparative studies to demonstrate clear advantages of a certain method while many of the classic procedures are nowadays frequently performed using a minimally invasive approach (endoscopic, laparoscopic or percutaneous). This paper presents an overview of the current available methods for the treatment of the pancreatic pseudocysts emphasizing the advantages, disadvantages and the limitations of each method – including the need for specific equipment and availability of highly trained medical specialists. The ideal management of the pancreatic pseudocysts involves a multidisciplinary team with all the possible methods available but this is rarely possible in the current medical practice.
