Trasmisión vertical de la enfermedad de Chagas: reporte de caso 查加斯病的垂直传播:病例报告

BACKGROUND: Chagas disease is a zoonosis caused by Trypanosoma cruzi, a parasite endemic to Latin America. Children become infected by contact with vectors or by congenital transmission. CLINICAL CASE: Illiterate indigenous patient, 32 years old, from the rural area of western Colombia, with a history of 7 pregnancies and 6 deliveries, the current pregnancy with 29.5 weeks determined by the third trimester ultrasound, without prenatal controls, with preterm labor and severe oligohydramnios. The pregnancy ended by cesarean section, without complications and the birth of a girl who weighed 1290 g. At 50 days of life, he developed hemodynamic instability, respiratory distress, mucocutaneous pallor and fever with a diagnosis of sepsis of undetermined origin. Extension studies reported: positive microhematocrit parasitaemia for Trypanosoma cruzi, with amastigotes in the cerebrospinal fluid. Diagnosis: Chagasic meningoencephalitis. CONCLUSIONS: The vertical transmission of Chagas disease is low; congenital transmission is responsible for the progressive appearance of the disease in endemic and non-endemic areas. In pregnant women, it is important to include screening for Trypanosoma cruzi infection, as part of the prenatal control test suite .

OBJECTIVE: Evaluate the relationship between the pregestational body mass index and the gestational weight gain with the birth weight. MATERIALS AND METHODS: Cross-sectional analytical study. Through probabilis- tic sampling, 197 pregnant women and their neonates participated, whose delivery was attended at the San Juan de Lurigancho Hospital, 2018.The pregestational body mass index was classified as low weight, normal, overweight and obese; the gestational weight gain in insufficient, adequate and excessive; and the birth weight in grams. The variation of birth weight according to pregestational body mass index and weight gain were evaluated using the Kruskal-Wallis and ANOVA H tests, respectively. Similarly, a multiple linear regression model is required, a 95% confidence level is considered. RESULTS: The patients who started the pregnancy with obesity and low weight pre- sented neonates with greater birth weight (3516.9 ± 480.7 g and 3564 ± 148.5 g, respectively), it was determined that there is not association between the pregestational body mass index and the birth weight (p = 0.753). However, the birth weight was higher in postpartum women with excessive weight gain (3582.9 ± 442.1 g) and lower in those with insufficient weight gain (3278.9 ± 447.9 g), infantil futuro. 14,15,16 Por el contrario, la ganancia de peso insuficiente durante el embarazo es más propensa a que los hijos nazcan con bajo peso y sean pequeños para la edad gestacional. MATERIALS AND METHODS: Retrospective, descriptive, comparative and cross- sectional study carried out between January 2011 and January 2018. Inclusion criteria: patients who had a cerclage and the pregnancy ended in our unit. Study variables: age, BMI, pregnancies, abortions, days of gestation earned, days in which they were placed, weeks of gestation completed at the end of pregnancy, surgical time, medica- tions (tocolytics and antibiotics), complications from the placement of the cerclage until the end of pregnancy. For the descriptive analysis the statistical package IBM SPSS (version 22) was used; the distribution was obtained with Student's t test, the Shapiro-Wilk normality test was applied and the variables with normal distribution were analyzed, comparatively, with ANOVA and those with abnormal distribution with the Kruskal-Wallis test. days with the end of pregnancy at 36 ± 2 weeks. CONCLUSIONS: In patients with cervical incompetence cerclages are an option to prolong pregnancy. The Shirodkar technique had higher safety margins until the end of pregnancy (more than 34 weeks). The behavior in multiple pregnancies is similar, so in case of cervical incompetence should be applied.
